We are seeking dynamic, enthusiastic and motivated individuals to lead and work within our Older Adult Inpatient Wards. We have vacancies for Band 5 registered nurses on our Older Adult Inpatient Wards. The wards are located at Grenoside Grange Hospital and Michael Carlisle Centre in Sheffield.

This is an exciting opportunity for the post holder to be pivotal in the future development of our older adult care services. Working in close collaboration with other services, the successful candidate will be able to shape and mould the future pathway and function of our Elderly services.

Main duties of the job

You will need to be a Registered Nurse with experience of working within a multi-disciplinary team.

You will possess excellent communication skills in order to work in collaboration with the service user, other professionals and wider services, to deliver excellent person centred care. You will also have effective skills in assessing, planning, evaluating and managing care together with risk assessment and management skills.

You will need to be a good team player, have a positive attitude with a can do approach, to caring for the service user population. An understanding of the Mental Capacity Act and recovery models of care is desirable.

Successful candidates will also be expected to work a full 24hr shift pattern to meet the needs of the service.

Clinical & Patient Care
  • To ensure that comprehensive nursing & risk assessments are carried out for all assigned patients, reflecting patients identified needs.
  • To ensure that each patient has a comprehensive, clear and up to date plan of care, working in collaboration with the patient, relatives/carers, to meet the individuals identified care needs.
  • To ensure that patient care is delivered in a manner consistent with local & NICE guidelines
  • To have a good knowledge of the Mental Capacity Act and how it is used.
  • To take a positive approach to responding to patients in acute distress, adopting principles of diffusion and de-escalation. Where physical interventions or restraint are required, to approach in line with available guidance and approved techniques, providing for the safety and dignity of the patient(s). When acting as the senior nurse present, to direct the actions and interventions of staff in responding to the incident in line with expected practice.
  • To take positive approaches to patient engagement. & occupation. Where undertaking specific observations of patients, to do so in line with the Trusts policies and guidelines
  • To ensure that an appropriate therapeutic environment is maintained within the ward & person centred care is delivered at all times
  • To ensure that practice in relation to medicines is in line with local guidelines, individual professional requirements and responsibilities, and ensures individual patient involvement, engagement and concordance
  • As a member of the Team, to contribute to providing a quality environment for patients in relation to safety, dignity, comfort and day to day activities. Take active and positive approaches to supporting and encouraging patients with their dietary needs, over the 24-hour period
  • Assist medical staff with routine examinations
  • To ensure physical assessments of patients are carried out in line with local guidelines, individual professional requirements and responsibilities.
  • To respond to spillages of waste or bodily fluids in line with health and safety guidelines and control of infection guideline
  • To use relevant information in support of patient care and care planning, including available electronic information systemsProfessional
  • To audit care plans on a regular basis using an established audit tool.
  • To ensure that the care given is regularly and thoroughly evaluated against the objectives in order that the effectiveness of care plans can be maintained.

  • Following suitable Preceptorship, to actively develop skills andexperiences in relation to; functioning as the nurse in charge and shift co-ordinator risk assessment processes risk management planning
  • To participate with colleagues in the promotion of an active learningenvironment in order to maintain and develop high standards ofprofessional and clinical practice.
  • To support the development of junior staff and learners, through effective role modelling, mentorship (for students/ learners), supervision and professional leadership.
  • To actively participate in own supervision, annual appraisals and personal development planning
  • To undertake mandatory training, over the required period of time, and additional training as identified in support of clinical and professional developmentManagement & Team Working
  • Take charge of the ward, occasionally, in the absence of a more seniornurse.
  • To ensure that during any period of a shift when in charge of medicines, the custody of all drugs is in accord with the Trust Policy and procedures
  • To exercise proper and effective leadership within the shift team.
  • To contribute to and participate in developing approaches and initiatives relating to clinical governance, audit and service development
  • Effectively communicate with all colleagues to ensure co-operation in all areas of work
  • To be aware of the Wards Personal Safety Plan and apply in day to daypractice
  • To understand relevant Trust policies and procedures, applying them in practice and informing senior colleagues of issues arising from theirimplementation
