The Clinical Lead will lead a team of Registered Nurses and Health Care Support Workers and their deployment in the delivery of a clinically effective Nursing Service.

Responsible for the pro-active case management of a wide range of patients many of whom have complex needs.

Accountable to the Health Board Nurse Director and the Nursing and Midwifery Council (NMC) to provide safe, high-quality care to patients by implementation of policies/ standards and guidance produced both national and locally. These will include from NMC, Welsh Government, Powys Teaching Health Board, and others.

Job description Job responsibilities

The person appointed will be expected to undertake a range of duties to include:
Leadership and Management
Promote and ensure through ongoing review that all patients are treated in line with the principle of dignity and respect and that this is embedded within the team.

Work within and adhere to the NMC code of conduct; standards and guidance at all times, ensuring that issues of concern are escalated appropriately. Act at all times as a role model, presenting a professional approach and ensuring the standards for professionalism are set and displayed within the team. Provide professional advice to nurses within the team.

Monitor standards of practice within the team and manage poor practice as appropriate as defined by the NMC and HR policy.

Support the implementation of the Corporate Plan and Nursing and Midwifery Strategy, ensuring that all members of the team have clear roles, responsibilities, and lines of accountability.

Undertake a key leadership role for nursing within your area of responsibility, ensuring that registered nurses are empowered and enabled to assess, plan, implement, and deliver the highest standards of care to patients.

Initiating therapeutic treatments to reduce dependency and maximise the person's independence, in collaboration with patients, carers and other health and social care professionals.

Actively involved in team meetings and facilitate own team meetings to ensure flexible working and service provision. Provide and circulate minutes and all relating documents.

Responsible for the efficient use of resources within the budgetary allocation.

Line management responsibility for the team of nurses and support workers.

Recruitment and appointment of staff, absence management and the deployment and allocation of duties. Ensuring all registrants have active registration with the NMC.

Responsible and accountable for an identified case load, proactively promote health enhancing activities and behaviours, working with the patient and the MDT team to reduce unplanned hospital admission and coordinate care across all settings of the care pathway.
Quality, Safety and Effectiveness
Consistently work with the team to ensure standards of patient care are high and maintain safety and quality of the patient experience.

Ensure that lines of communication are maintained with the key working relationships to demonstrate politeness, courtesy, compassion, and sensitivity, promoting a positive image of the Health Board.

Provide information, knowledge, support, and training on a range of complex care conditions, treatment options and outcomes. Supporting patients in order that they can make informed choices.

Support the identification, investigation, and implementation of learning from concerns within the Health Board. This may necessitate a lead role as defined by the Lead Nurse and require investigation and report writing.

Apply the principles of Being Open in communicating with patients, families

and appropriate others in all aspects relating to concerns.

Ensure that information technology is utilised appropriately by all team members (WCCIS, Datix, WPAS) to improve patient care.

Take a lead role within your team in relation to Continuing NHS Healthcare, ensuring all patients individual needs are considered and when appropriate the completion of the Decision Support Tool is undertaken to a high standard, supporting the local process through attending panels as requested.

Within the team, lead the implementation and monitor local and national nursing policy, procedures, and guidelines to support best practice and ensure they are adhered to together with current legislation.

Take a lead role in ensuring that the environment of care is appropriate for the nursing activities being undertaken, ensure adherence to infection control policies are maintained, act on audit results and quality improvement initiatives to ensure that improvements are made in a timely manner.

Support the effective transfer of patient care to and from your case load by ensuring that appropriate assessment, communication, and liaison thus facilitating safe admission and discharge. Work specifically with Carers to ensure an equal partner approach to ongoing care.

Using expert knowledge of managing and delivering care for patients with highly complex and unstable health needs within the community setting and ensure the delivery is safe, efficient, and effective.

If an independent prescriber would need to prescribe ensure it is in line with the Prescribing Formulary, Health Board/ All Wales/ NMC policies and guidance and making clinically and cost-effective prescribing decisions.

Take a key leadership role in implementing quality improvements into the area of responsibility, including Quality Improvement programme areas. Take a zero-tolerance approach to practice which does not meet the highest standard

working with staff/teams to consistently improve patient outcomes and experience.
Nursing Workforce Development and Team Working
Responsible for the accurate recording, timely collection, completion, submission of relevant information to support patient care, operational management, nursing staff and skill mix, service development and audit, analysing and interpreting this information to inform leadership within your area of responsibility.

Ensure that all nursing staff receive regular Individual Performance Reviews and have Personal Development Plans in place centred on delivering services focused on patient/service need.

Empower all staff to develop their roles where supervision, mentoring and coaching each other and students is the culture within their area of responsibility.

Undertake a training needs analysis for nursing within your team, ensuring clear prioritisation and including mandatory training needs.

Implement agreed training plans, maintaining accurate records of progress made against the plan.

Actively participate in the research and development agenda, focusing on the improvement to services as a result. Promote the development of nursing research projects within the locality e.g., 1000 Lives Plus.

Consistently develop skills, knowledge, and expertise to provide appropriate patient care in line with current service development e.g., clinical assessment.

Work with the MDT and appropriate partners to ensure that appropriate teaching and health promotion programmes are developed and coordinated within community setting to meet the local health needs.

The post holder will be expected to participate in a performance system in operation or introduced within the Health Board.

A requirement to maintain clinical skills as required for care area. This may incorporate independent nurse prescribing dependent upon appropriate qualification and training.

Person Specification Qualifications and/or Knowledge Essential
  • Live registration with the NMC as Registered Nurse
  • Degree/Diploma in Nursing
  • Post registration qualification / significant experience in Mental Health
  • Management/ Leadership qualification or equivalent preparation/experience
  • Knowledge and skills to manage patients with complex needs
  • Knowledge of current Nursing issues within NHS care systems
  • High level of knowledge of NMC code of conduct and professional standards/guidance

  • Independent Prescribing Qualification
  • MSc Advanced Clinical Practice
Experience Essential
  • Experience at management level demonstrating ability to deliver improvements to service usercare
  • Experience of proactive caseload management of wide range of patients
Aptitude & Abilities Essential
  • Ability to use computer systems (email, word processing, spreadsheets)
  • Significant clinical experience in assessing, planning, implementing, and evaluating care for the patient group of the caseload
  • Well-developed verbal and written communication skills

  • Demonstrable ability and track record of acting with care, compassion, promoting dignity and respect, role modelling the highest standard of professionalism
