The Adult Community Mental Health Teams are multi-disciplinary teams that offer assessment, treatment and care to adults who experience complex mental health difficulties.This is a leadership role and the post holder will work closely with the Team Manager to achieve positive outcomes in the teams' clinical management

Main duties of the job

o Providing day to day clinical leadership ensuring the highest standard of care is maintainedo Identify training needs for the team and enable access to trainingo Identify the skill mix required for staff recruitment and support this processo Audit case notes and care plans against approved audit tool.o Liaise/have working relationship with other Trustwide teams and external agencies.o Support the planning, delivery and evaluation of interventions and treatments where there are complex issues and/or serious illness

Provide specialist knowledge and advice to the team and partnershipagencies relating to mental health services.o Undertake Care Co-ordination responsibilities for a designated reduced caseload of individualso Support and supervise staff to enhance the skills, knowledge and expertise of the team in line with current evidence based research practice, training and modellingo Have a clinical role in the team.o Provide support to the team in regard to the assessment role.o Take a lead role when liaising with GPs, Primary Care Services and otherexternal agencies who refer into the Adult Community Mental Health Teamso Ensure and maintain both the national and locally agreed clinical andprofessional standards are embedded within the service.o Maintain the agreed outcomes of the service.o To deputise in absence of the Team Manager.

To maintain and develop own and the teams clinical skills, knowledge andprofessional practice via a local training template in line with professional,NICE guidelines and service need, ensuring the maintenance of effectiveevidence based practice. To actively contribute towards the organisation's clinical governance systems, taking responsibility for quality standards and work towards the continuous improvement in clinical and service quality. To observe confidentiality in all matters relating to service users and staff. To adhere to Trust policies and procedures and support staff to access and work within specific policies that relate to their employment. To participate in regular clinical and operational supervision. Work within NMC Code of Conduct and ensure professional registration and revalidation is maintainedClinical Responsibilities To act as a clinical leader providing support/advice to GP's and others. To act in a consultative role for staff, supporting the assessment and caremanagement of clients referred into the service. To be able to assess peoples complex health and well-being needs andidentify the appropriate interventions to meet those needs. To develop and implement comprehensive care plans including riskmanagement plans. To support the investigation of complaints and RCA incidents To lead on the development, implementation and monitoring of standards. To develop and implement effective policies and procedures. To develop robust risk management standards, develop and monitoreffective therapeutic risk within the service. To ensure the clinical environment is conducive to learning To act as a mentor assessor for student nurses, and contribute to studentnurse and other community organisations training needs To provide educational material for service users, family members and other professional groups To be conversant with and practice within the realms of the 1983 MentalHealth Act. To engage in clinical supervision both as a provider and receiver To be responsible for ensuring the maintenance of clinical standardsregarding assessment of care needs. Work with the Team Manager to ensure equitable use of all resources tosupport an effective service. To act as a lead point for the instigation and continuation of CQC standards, infection control standards, National patient safety standards and standards for better health. To manage a reduced caseload To ensure allocation and staff caseloads reflect their level of experience and competence to meet the needs of service usersEducational and Research Responsibilities Develop internal learning processes to evaluate and monitor the assessment process. To participate, and where appropriate, lead on relevant research projectsrelating to the service To ensure clinical practice develops through the critical analysis andevaluation of evidence based research findings To be recognised as Clinical Lead and act as a local resource point todisseminate information and be available for consultation engaging a widenetwork of professions, voluntary, employment and education regarding the impact of mental illness.Management Responsibilities To organise and attend relevant steering groups and forums appropriate to the development and maintenance of the team. To improve and evaluate the effectiveness of the service through feedback from service users and other partner agencies To conduct Personal Development Conversations/appraisals within the team, being aware of the particular developing needs of staff To assist in the recruitment and selection of staff. Ensure prescribed medication are appropriately managed and stored Lead and participate in investigations of complaints, concerns and seriousincidents Assist in the effective management of financial budgets and resourcesGeneral Ensure compliance with Health and Safety issues within the sphere ofresponsibility To carry out other duties as may be appropriately requested to pursue the objectives of the Mental Health Partnership.Communication To be responsible for promoting the service within Health and Social Care economy and wider partnership. To promote liaison and inter agency working and collaboration To be responsible for the development and maintenance of effective lines of communication on complex matters, issues and ideas, between staff,management and partner agencies. To represent the team at appropriate service meetings and ensureinformation is cascaded to the team To be proficient in the use of electronic and information technology systemsLeadership Skills To promote the ethos of a flexible service, that places service users and their family at the centre of the care package To promote the values and ethos of the service in line with the principles and the policy implementation guidelines.   • oRMN (Mental Health Level 1)
  • oDegree or evidence of post registration study in subject relevant to specialty
  • oEvidence of post registration development and training
  • oExperience of working in multi-disciplinary clinical teams
  • oKnowledge of Trust values and the ability to embed these in day to day practice
  • oDemonstrate an understanding of national government initiatives in health and social care and how they impact on local services
  • oDemonstrate understanding of current developments in the NHS and their implications for leadership and quality care for service users
