We hope that you will join us as our Band 5 Community Learning Disability Nurse, supporting the delivery of the new Intensive Therapeutic Short Break Service (ITSBS)across the Pennine Care footprint.

This is a Community Learning Disability Nurse role within a new multiagency team comprising clinical psychology, learning disability nursing, speech therapy, occupational therapy and support workers. The Intensive Therapeutic Short Break Service sits alongside the Community Learning Disability and Child and Adolescent Mental Health Services across the footprint, consisting of community learning disability nursing, clinical psychology and psychiatry

The team will be working with a small number of children and young people who have severe learning disabilities and who have been identified as having complex needs including autism and/or behaviour that challenges, that require intensive support. Working alongside the local respite and short breaks services, the Intensive Therapeutic Short Break Service will work together to create, instigate and support person centred positive behaviour support plans with the aim of improving the quality of life for this group of children and young people and reduce any restrictions or restraints (in accordance with NICE guidance).

Main duties of the job

You will have experience working with this client group and have developed some skills in assessment, formulation, intervention and evaluation. You will have experience of working in multi-agency settings. Due to the level of independent working and management of risk, you are expected to have experience of working with this client group.

Person Specification EDUCATION / QUALIFICATIONS Essential
  • Relevant health or social care qualification with current professional registration e.g. o R.N.L.D o BSC Hons in Learning Disability Nursing and Social work or equivalent relevant experience
  • NMC REGISTRATION
Desirable
  • Post registration qualifications in related subjects e.g. Positive Behavioural Support, Community Specialist Practitioner, Autism
  • COMPLETION OF CYP IAPT LD
Experience Essential
  • Experience working with children and young people with learning disabilities and complex needs (including Autism, mental health needs and behaviour that challenges) and their families in multi-agency settings
  • Experience of using principles of positive behavioural support and developing PBS plans
  • Experience of administration and organisation for caseload management whilst prioritising demands
  • Experience of multi-disciplinary and multi-agency team work
  • Experience of consulting with other professionals
  • Experience managing risk in children with learning disabilities and challenging behaviour
Desirable
  • Experience supervising therapy assistants
  • Experience of functional assessment and analysis

  • Understanding of Positive Behavioural Support Principles (PBSP)
