An exciting opportunity has arisen for a qualified Learning Disability Nurse to join the Wirral Community Learning Disability Team, which provides multidisciplinary care to people with learning disabilities, from its base at Leasowe Millennium Centre. The CLDT is co-located with social care and has excellent working relationships with the ICB meaning that good multiagency working is achieved.

The CLDT is part of Cheshire and Wirral Partnership NHS Foundation Trust Learning Disabilities, Neuro Developmental & Acquired Brain Injury Care Group, and has excellent governance structures. We have strong links to local universities as a Trust, and take an active role in leading in Clinical Research.

We require a practitioner with considerable experience, who can work alongside the wider Multi-Disciplinary Team and leadership in relation to ensuring high quality evidence based and innovative practice.

Person Specification Experience Essential

• Experience/ability to work independently.
• Excellent knowledge of the physical health agenda, relevant legislation and government policy.
• Ability to deliver training and make formal presentations to groups.
• Experience of working with people with a learning disability in a community setting.
• Direct experience of working with clients with a learning disability and complex needs i.e. challenging behaviour, autism, mental health problems
• Plan/Evaluate care to clients with complex needs
• Ability to take lead health coordination function for people with complex health needs.
• Ability to undertake complex risk assessments and manage risks through coordinated multi agency approach to care.
• Ability to design and deliver training and make formal presentations to groups of professionals, service providers, service users and families
• Ability to support service users, their families and carers in crisis situations.
• Audit and benchmarking skills.
• Demonstrates strong leadership skills and the ability to work across organisations to obtain positive outcomes for service users.

• Registered learning disability nurse (RNLD; RNMH) with a good amount of experience at band 6 level
• Hold a relevant degree e.g. Specialist practitioner or equivalent.
• In-depth experience, evidence of post registration training at degree/masters level e.g. - Complex physical health - Management leadership - Clinical audit & research
• Demonstrates commitment to ongoing training and personal development

Desirable

• Leadership or management qualification.
• Evidence of post registration modules at degree/masters level e.g. - Mentorship module.
