Droitwich, Ombersley and the Rurals Neighbourhood Team are looking to recruit an enthusiastic and committed occupational therapist to be based either in Droitwich medical centre or Tenbury community hospital

If you are looking for a role where you can use your skills to make a difference come and join Worcestershire Community Teams where Nurses, Physiotherapists, Occupational Therapists, Health Care Assistants, Advanced Clinical Practitioners, Nursing Associates and Administrators work together in local Neighbourhoods and alongside GPs to support people to live in their own homes.
Main duties of the job
Work as an autonomous practitioner within an interdisciplinary Neighbourhood team providing rapid and expert professional assessment and treatment interventions to patients either in their own homes or other community settings to avoid inappropriate hospital admission or to facilitate early discharge.. This will be achieved through a combination of hands on expert clinical skills as an Occupational Therapist and knowledge to treat, educate and support patients and by working with patients and their families to assess, plan and implement appropriate packages of care and treatment
As part of the Neighbourhood team, the post holder will also triage patients through the single point of access/triage to ensure prioritization of clinical need. They will develop and co-ordinate individualized care and rehabilitation plans in close liaison with the team, the patient and their carers as well as all other relevant services

Person Specification Qualifications Essential
- Degree / diploma in Occupational Therapy
- Current registration with HCPC
- Evidence of relevant post graduate courses / Continual Professional Development maintained in portfolio
- A range of post graduate experience including work in a variety of physical and/or mental health settings / specialities
- Experience of working in a multidisciplinary / interdisciplinary team
- Recent experience in a community setting
- Experience of caseload management and prioritisation
- Experience of working in an integrated team
- A range of evidence based Occupational Therapy clinical skills and knowledge appropriate to specialism
- Good level of knowledge relating to disability, equipment and adaptations
- Ability to communicate clearly and effectively in English in both verbal and written formats
- Ability to work proactively with other agencies to prevent hospital admission and facilitate early discharge
