Access and Assessment Team Community Mental Health Practitioner
£3k Welcome bonus included ( T&Cs apply)

Our Great Yarmouth and Waveney Access and Assessment Team (AAT) are looking for a Band 6 Community Mental Health Practitioner. AAT is the front door to all our adult mental health services in Great Yarmouth and Waveney. We are now embarking on an exiting new venture with our partners, working towards a truly integrated service model. We are actively involved in the community transformation project locally and would encourage new staff to be part of the development of the service.

To apply for this role you will need to be a qualified professional Nurse, Occupational Therapist or Social Worker with experience of working in the community supporting working age adults with mental health difficulties. The successful candidate will need to have a sound understanding of the Care Programme Approach and be able to demonstrate their abilities in risk management. You will be part of the triage and assessment process managing referrals from our GP surgeries and other local services.

We are looking for professionals who are caring, supportive and reflective in their practice. You will be joining our friendly team of practitioners, have access to regular supervision and training opportunities.

Main duties of the job

  • To undertake the assessment, planning and delivery of direct patient care with minimal supervision, ensuring continuity of practice and working within a multi-disciplinary and multi-agency environment.
  • To communicate decisions about mental health care services, referrals and future support arrangements to people with a mental health care need.
  • To be responsible for accurate recording of all patients support, including assessments, reviews, day to day contact and discharges.

  • Nursing degree or equivalent, Dip OT, Clinical Psychology or equivalent
  • Higher degree or post graduate certificate in area of clinical / therapeutic practice

  • Good to great management course, or equivalent

Experience

  • Able to demonstrate appropriate level of knowledge of mental health practice from previous experience
  • Demonstrate effective risk assessment and risk management experience

Norfolk and Suffolk NHS Foundation Trust is the main provider of specialist mental health services for the counties of Norfolk and Suffolk, serving a population of 1.6 million people.
