The Riverside Neighbourhood Team, St Johns House Medical practice, Bromyard Road, Worcester is looking to recruit an Allied Health Professional on full/part time basis who are innovative/keen to join the team, supporting to deliver high quality care, and continue development of the Intensive Support Team in the community providing short term care on discharge from hospital/to prevent hospital admission, assisting recovery & facilitating rehabilitation. You will be responsible for rapid assessment & treatment of patients who have urgent/complex physical /mental health/social care needs to avoid inappropriate hospital admission/facilitate early discharge. You will work alongside community nurses, HCAs, ACPs & GPs to provide compassionate care.

The successful candidate will provide guidance and supervision for junior staff and Health Care Assistants.

You will work on a roster basis. The team operates from 07:30-18:30 each day and you will work 07.30-15.30, 08:30-16:30 or 10:30-18:30 shift patterns.

Main duties of the job

  • Rapid assessment and treatment of patients who have urgent and/or complex physical /mental health and social care needs to avoid inappropriate hospital admission or to facilitate early discharge.
  • Co-ordinating the intensive support caseload to ensure patients receive responsive high quality care/rehabilitation that is goal centred to facilitate early discharge from hospital and/or prevent hospital admission
  • Liaising with other members of the multidisciplinary team to ensure timely discharge from the caseload
  • Leading the weekly multidisciplinary meeting
  • Delivering a high standard of occupational therapy/physiotherapy assessment and treatment to patients in their own home
  • Provide advice on the rehabilitation management of patients to families, carers and other members of the MDT
  • Work with a wide variety of presentations including falls, orthopaedics, neurological conditions, musculoskeletal, and palliative/end of life patients

Person Specification Qualifications Essential
  • Degree / diploma or equivalent in Occupational therapy/Physiotherapy
  • HCPC registration
  • Evidence of relevant post graduate courses / Continual Professional Development maintained in portfolio
Desirable
  • Relevant validated clinical course
  • Clinical educators qualification
Experience Essential
  • A range of post graduate experience including work in a variety of physical and/or mental health settings / specialities
  • Experience of working in a multidisciplinary / interdisciplinary team
  • Experience of caseload management and prioritisation
Desirable
  • Experience working in a community setting/integrated team
  • Experience of supervision of junior staff or students
  • Experience of implementing change in clinical practice / audit
  • Experience in rehabilitation settings
Knowledge Essential
  • A range of evidence- based Occupational therapy/Physiotherapy clinical skills and knowledge appropriate to the role
  • Knowledge of rehabilitation concepts and services
  • A good level of knowledge in a wide range of physical and mental health conditions
  • Good level of knowledge relating to disability, equipment and adaptations
  • Knowledge of IT skills
