This is a varied role working as part of a multi-disciplinary team the PCMHP will offer assessments, brief treatments and onward signposting advice to patients within a primary care setting assisting GPs and Health in Mind to manage the demand of mental health presentations within the primary care setting.
The post holder will be offering input into the Triage function for the service, screening referrals and risk assessing based on the presenting need to decide the appropriate care pathway.
Main duties of the job
The post-holder will be a member of a GP led primary care team, and the 'Health in Mind' service. Core functions will be to undertake screening and assessment of patients with mental health problems referred by GP's to determine the most appropriate intervention and offer support and advice.
The post-holder will work closely with colleagues and stakeholders including supporting GP's and primary care teams in assessing and managing risk.
The PCMHP focuses on ensuring the patient gets the right care and the right time focusing on the implementation of patient-centred care packages.
There is also an opportunity where required to provide supervision for students and other mental health trainees, and training to colleagues in the Health in Mind service.

- Include relevant professional registration
- Extensive post-qualifying experience of clinical work with people with a range of mental health problems
