York Care Homes and Dementia team for older people services provides dedicated input into all the care homes in the York and Selby Localities. The team therefore provides dedicated input into 50 care homes covering a bed base of 2154 beds. The team receives 563 referrals in the last year. The Care Homes team provide 8am- 8pm service on all 7 days of the week.

a. York Care home and Dementia team was formed with holistic care provisions in mind for care home residents needing mental health input. Primary aim of the team is to proactively manage care home residents needing acute mental health input and enabling community-based care b closely working with primary care colleagues and other Multi-disciplinary team members such as social workers and district nurses involved in patient care. In this context care homes and dementia team function is vital in terms of managing preventable admissions to mental health organic wards. The team also works closely with primary care colleagues in terms of reviewing the risk of polypharmacy particularly antipsychotic review in care homes patients.

Main duties of the job

The consultant will not hold a case load but be expected to work to a clinical leadership model of consultation. It is expected that the Consultant would need to complete 10-12 reviews every week.

Person Specification Qualifications Essential
  • MB BS or equivalent medical qualification.
Desirable
  • Qualification or higher degree in medical education, clinical research or management.
  • MRCPsych qualification
  • Additional clinical qualifications.
Experience Essential
  • Excellent knowledge in specialty
  • Excellent clinical skills using bio-psycho-social perspective and wide medical knowledge
  • Excellent oral and written communication skills in English
  • Able to manage clinical complexity and uncertainty
  • Makes decisions based on evidence and experience including the contribution of others
  • Able to meet duties under MHA and MCA
Desirable
  • Wide range of specialist and sub-specialist experience relevant to post within NHS or comparable service
