Solent East CAMHS is a specialist mental health service that work alongside children aged 0-18 and their families to address their mental health needs and improve their day to day lives. We are based in the vibrant and dynamic city of Portsmouth and our CAMHS Liaison Practitioner will assist in the provision of care and treatment for the young people who have presented in acute mental health crisis at QA Hospital in Cosham.

The service serves a socially and economically diverse population. It benefits from strong working relationships across many agencies and regional areas. Main duties of the job

• Undertaking rapid crisis assessments for young people referred through the general hospital and, providing short crisis intervention and signpost to other specialist services as required
• Contributes to effective discharge planning and referral processes to health care providers and support agencies to meet identified health needs.
• Based on assessment, to formulate & convey sensitive, complex, & sometimes unwelcome information to clients & other professionals and manage this as constructively as possible.
• To support senior practitioners completing complex assessments, though the gathering of information, completing referrals and liaising with partner agencies
• To work alongside senior practitioners, in providing and sign-positing to specialist advice, specifically relating to child and adolescent mental health, to professionals in other agencies/organisations working in the field of child and adolescent mental health
• Accurately documents assessments of patients/client's health status, diagnosis & response to nursing & other interventions and treatments on the Trust Clinical Records System
• Identifies when a child may be at risk and will refer to the local multi- agency safeguarding hub when appropriate
• Practices in accordance with legal, ethical, culturally safe, & professional standards including the Code of practice

Communication

• Work collaboratively across the CAMHS directorate and partnership agencies.
• To liaise with and where appropriate participate in joint work with other specific teams.
• Work collaboratively with senior members of the team in decision making and formulation of risk.
• Developing and maintaining positive working relationships with all internal and external partners and stakeholders.
• To ensure documentation and care plans are comprehensive and of the highest standard in line with Trust policy.

• Identifies situations of clinical risk and takes appropriate action to ensure a safe environment for patients/clients/families and staff.
• Contributes to evidence-based nursing practice in speciality areas.
• Demonstrates commitment to quality improvements, risk management and resource utilisation participating in monitoring and evaluation activities including audit and research activities
• Provides input into clinical standards/protocols and policies and undertakes clinical audits as required.
• Evaluates the effectiveness, efficiency and safety of clinical practice contributing to service improvement initiatives.
• Participates in the implementation of nursing models of care appropriate to patient/client population needs.
• Contributes to and participates in Solent NHS policy development providing own expertise where relevant and feedback on proposed polices based on relevant experience and expertise where applicable.

• Fully adheres to the Solent NHS trust Integrated Clinical and Safeguarding Supervision policy along with Safeguarding Children and adult policies, and information sharing protocols to ensure the health and well-being of children and adults at risk.
• Familiarity with the requirements of the Mental Capacity Act and maintains MCA mandatory training.

Person Specification Qualifications Essential

• Professional Qualification (Nursing / Occupational Therapy / Social Work)
• Professional Registration (NMC / HCPC)

Experience Essential

• Experienced in developing care plans with young people and their families
• Experience of specialist mental health assessment and treatment of children and adolescents and their families/carers where appropriate as a full member of a multi disciplinary team located in community, primary care, outpatients or inpatient settings
• Experience of working with a wide variety of children, adolescents and their families/carers across the whole range and presenting problems that reflect the full range of clinical severity.
• Experience of MDT working
• Experience of teaching, training and / or supervision
• Evidence of recent CPD

Desirable

• Post qualifying experience of working as a specialist mental health worker within a mental health setting
