An exciting opportunity has risen for two Band 5 Community Mental Health Nurses to join Community Rehab and Recovery team. We are looking for an enthusiastic and dynamic individual who is able to work with a wider MDT, working alongside other professionals and striving for best outcomes for service users working across 7 days a week 8 am -6pm with enhancements on a weekend, intensive work, working alongside the Care coordinator or Lead Professional. We are happy to support newly qualified nurses with their preceptorship and you will have a thorough induction programme offered to join Community rehab and Recovery Team Durham and Darlington. This will be a developmental role into a Band 6, working intensively with the Band 6s within the Team. You will be joining us at an exciting time of planning and implementation of the Community Mental Health Framework.

As a core member of the team, you will also be responsible for: -
  • your own caseload
  • providing assessments
  • delivering intervention in the community.

You will ensure that the service users' needs are: -
  • identified and central to their CPA Well-being and Recovery plans
  • promoting social inclusion
  • independence and well-being

You will demonstrate:-
  • Excellent assessment skills
  • interpersonal and team working skills
  • be able to work collaboratively with a range of statutory and third sector.

Main duties of the job

To improve the lives of people with mental ill health by minimising the impact of their condition through the delivery of excellent services to promote recovery and wellbeing.

To be compassionate in meeting the needs of patients and their carers.

To achieve successful completion of the Preceptorship Program.

To work as part of a community team which may be integrated, working collaboratively and in partnership with local authority services.

To be professionally accountable and responsible for patient care undertaking a range of clinical/therapeutic interventions in patients' homes and other community settings.

To act as lead professional or care coordinator as appropriate.

To promote social inclusion, community access and participation through the provision of patient care.

To facilitate others in the team to develop competence by providing leadership, day to day supervision, clinical advice and clinical supervision to associate practitioners, community support workers and students as appropriate.

To be committed to and involved in activities integral to the Trust's Quality Improvement System.

To promote at all times a positive image of people with mental ill health.
  • To promote at all times a positive image of the Adult Mental Health Service and the wider Trust.

This post is only open to those candidates who already have their NMC registration in place and have successfully completed the preceptorship programme. Please note: Newly qualified applicants or those due to qualify will be recruited via a different process.

Person Specification Qualifications Essential
  • RNMH current registration with NMC
  • Evidence of continuing Professional development
  • MIP, FLIP, ENB998 Teaching and Assessing in clinical practice or city and Guilds 730 Teaching in adult Education or Willingness to work towards . Qualification must be achieved within a time scale .
  • Trained clinical Supervisor (Within agreed tine scale)
  • Key skills in Literacy, Numeracy and ITQ level 2 or Equivalent
  • Postgraduate Qualification in relevant area
Experience Essential
  • Working with people with mental ill health.
  • Working in a therapeutic organisation
  • Working in a community Setting
  • Working collaboratively with service users with their families and carers .
  • Working in an MDT
  • Working in partnership with other agencies.
Knowledge Essential
  • Evidence based knowledge of based practice in caring for patients in designated field.field
  • Understanding of legislation .
  • CPA and its application in practice.
  • Understanding Safeguarding and its application in Practice.
  • Clinical risk assessment and management and its application in practice.
  • Clinical governance and Clinical Supervision.
  • Research and development Methodology.
  • Awareness of Psychological models of treatment and Care.
