We are looking for a qualified or Allied Health Professional, with Older Peoples Mental Health experience to support the Memory Service providing enhanced clinical triage.

This exciting opportunity is a role with working from home opportunities. There is an expectation that you will attend a DPT base once a week- base to be negotiated.

We have clinics in North Devon, South Devon and Exeter and you will also provide clinical oversight of patient and staff queries where appropriate under supervision from the Devon Memory Service Community Team Manager.

Main duties of the job

This role involves triaging a high volume of routine referrals, and liaising with GP's, patients and carers mainly over the telephone.

Excellent communication skills and the ability to work autonomously are key.

Good experience of the dementia journey, and the ability to work well with patients and carers is essential.

Good experience and knowledge is required and the ability to work alone and sometimes under pressure, due to this being a small hybrid team that works remotely.

Person Specification Knowledge and skills Essential
  • An understanding of the Mental Health Act/ Mental Health Capacity Act requirements.
  • Understanding of community structures and Health and Social Care Agenda for Older Adults
  • Understanding of recovery principles in Mental Health
  • Understanding of the organic presentations in Older People
  • Excellent communication skills including the ability to engage with clients experiencing a wide range of mental health presentations.
  • Ability to communicate with a wide range of professionals from various settings.
  • Able to recognise potential difficulties and participate in finding solutions
  • Ability to assess and develop plans to manage risk.
  • Able to plan creatively to maximise effectiveness of care plans, including complex multi-agency care plans
  • Able to work independently and as part of a team.
Qualifications Essential
  • A professional mental health qualification and registration e.g. RMN, Social Work Degree or equivalent, BSc/Diploma in OT, SROT
Desirable
  • Chartered Clinical Psychologist, ENB 811/812 or Community Nursing Degree
  • Non-Medical Prescriber and/or another recognised therapy qualification
  • Post graduate qualification relevant to specialism
  • Clinical Supervision qualification
  • Training to level 3 in Safeguarding Adults and children and young people: roles and competencies for Health Care staff
Experience Essential
  • Excellent communication skills including the ability to engage with clients experiencing a wide range of mental health
  • presentations.
  • Ability to communicate with a wide range of professionals from various settings.
Physical Skills Essential
  • Ability to be mobile across a geographical area in a timely manner and in accordance with service need.
