The post holder is responsible for the assessment of care needs and contributing to; the formulation of the person's needs and the development of a relevant plan based upon the person's negotiated goals and relevant evidence based interventions. The post holder, inconjunction with the multi-disciplinary team, will provide nursing interventions to support the person to meet their agreed goals and coordinate and/ or participate in the review ofprogress towards those goals, utilising recommended outcome measures, involving the service user and any identified carer throughout the clinical pathway.

An exciting opportunity has arisen within our recently formed Integrated Mental Health Team in East Staffordshire (Burton on Trent). Integrated Mental Health Teams offer community based interventions for people experiencing a range of mental health conditions. The teams have been developed in line with MPFT's Community Mental Health Transformation Framework to offer integrated, place based community mental health services.

We are looking for a Mental Health Nurse/ Occupational Therapist to join our dynamic and friendly East Staffs Integrated Mental health Team.

The Integrated Mental Health Team is an innovative secondary care community team that is guided by collective leadership, providing evidence-based psychological and medical interventions to patients with a wide range of complex mental health needs.

The patient's journey is guided by psychological formulation of his or her presenting needs and difficulties, which subsequently inform the development of a plan of care for the patient. We draw from evidence-based interventions to provide high quality and effective individual and workshop treatments.

We are looking for a mental health nurse / occupational therapist who has a keen interest in working in a multi-disciplinary setting with patients experiencing mental health difficulties using a range approaches. The successful applicant will be motivated to provide high quality care to our patients.

About us

Maintain an up to date awareness of the infection control precautions relevant to your area of work and implement these in practice. As a minimum, this must include hand hygiene, the use of personal protective equipment, the use and disposal of sharps and communicating the importance to patients, prison staff and other health care staff you are working with. Details of the precautions and sources of advice and support to assess and manage infection control risks are provided through mandatory training which all staff must attend at intervals defined in the Trust policy on mandatory training and can be found in the Trust's infection control policies and national guidance, such as that published by NICE.

Person Specification Qualifications Essential
  • Registered Mental Health Nurse on NMC
  • Register or Occupational Therapist with HCPC registration
  • Conforms to NMC/HCPC requirements
Desirable
  • BSc degree in nursing or equivalent level
  • Registered mentor
  • Psychological intervention Qualification including EMDR, CBT, CBTp, Family Interventions, CAT, DBT
  • Phlebotomy training
Experience Essential
  • Experience of working with people experiencing mental health related difficulties
  • Experience of working with people who have difficulty in engaging with services and have complex mental health needs.
  • Experience of assessing, planning, coordinating and managing people with mental health problems in a community setting
  • Ability to access any local resources and to use networking where possible.
Desirable
  • Experience of working in community setting and making decisions autonomously
