We have an exciting opportunity for a Band 6 Occupational Therapist within the Community Mental Health Team (CMHT) for Older People in Sedgemoor.

We are looking for an exceptional candidate with an OT qualification that has both the experience and passion to work with older people with mental illness.

Main duties of the job

Sedgemoor CMHT for Older People is a Multi-disciplinary team that provides assessment, treatment, reviews and support for people with complex mental health problems. We link closely with other professionals such as GP's and District Nurses and other organizations such as Adult Social care and the voluntary sector.

As an OT, you will be expected to complete OT assessments- home safety assessment, Mobility assessments etc. Also run groups within the day hospital - memory strategies, CST, PALS.

As a Care Co-ordinator you will have a defined caseload and will focus on working directly with clients, families and their carers to ensure people receive an effective assessment with support and information to manage their condition whilst considering any possible risk or safe-guarding concerns . You will demonstrate exceptional interpersonal skills with the ability to communicate effectively whilst remaining sensitive and empathic.

Job Purpose:

- The post holder will deliver a high quality, comprehensive mental healthservice for older adults of with severe and enduring mental health difficulties both in Older Persons Mental Health Specialist Care Settings, the Community and those in Memory Assessment Service and day hospital.

- The role involves managing a defined caseload with a focus on working with individuals and their significant others who experience a complex range of needs and difficulties.

- Acting as a key member of the multi-disciplinary Team, the post holder will provide assessment, psychosocial, evidence based interventions using a collaborative, recovery approach and will be skilled at positive risk management. Liaison with a wide range of agencies is central to the role.

- The post holder will be based in the Older persons Community Mental Health team base either in Bridgwater or Burnham on sea, Somerset.They will have a caseload of Older People experiencing Mental Healthdifficulties.- They will offer specific assessments to the day hospitals.- They will take on the role of Care Co-ordinator if Occupational Therapy is the main intervention.- Complete Memory assessments.- They will complete OT assessments for Clients with a dementia in thememory service.- They will run, participate, and develop groups in the day hospital.
