The Home 1st and the Health Ageing Team are excited to advertise a new Mental Health Practitioner Post. These innovative posts will support the mental health needs of patients accessing Home 1st Services. These exciting new roles are partly funded by the Urgent Care Response Funding.

Home 1st is a multi-disciplinary team that provide a seamless service supporting early discharge and admission avoidance in the community.

Main duties of the job

You will be employed within the Healthy Ageing Team in Mental Health Services and receive regular clinical supervision from experienced Mental Health Practitioners and Managerial supervision from the team Manager of the Rapid Response Service. You will work within Home 1st which aims to help people remain at home by providing an admission avoidance and supported discharge service to adults in Milton Keynes.

In addition to the community-based services Home 1st has also developed the highly-successful 49 bed Seacole Virtual Hospital. You will work alongside Rapid Response Nurses, Physiotherapists, Occupational Therapists and a Clinical Psychologist to ensure that patient's mental health needs are assessed and appropriate intervention and support provided as part of their Home 1st treatment. The role will involve assessment and intervention with individual patients, liaison with other mental health services when appropriate and also training and support to staff in Home 1st.

  • To accept referrals from Home 1st clinicians where Mental Health expertise is required
  • To provide advanced holistic assessment and expert clinical care including short term monitoring of patients during an acute period of illness
  • Provide a specialist resource for people with an acute illness and support relatives and carers
  • Provide a specialist mental health resource for people using the service as required and offer support to their relatives and carers.
  • Provide mental health advice to community health colleagues in Home 1
  • To support and ensure smooth transition of the individual between community and Mental Health services where required.
