An exciting opportunity has arisen within the Halton Community Learning Disability Team for an Advanced Forensic Practitioner to assume a lead in providing effective clinical assessment and interventions to meet the complex needs of people with learning disability with forensic behaviour; particularly those whose behaviour presents serious risks to the community and/or brings them into the Criminal Justice System.

They will be based within Intensive Support Team (IST). The post holder will develop PBS intervention plans and risk management plans ensuring the service user, family / carers are able to understand and implement the interventions. They will, provide training to a wide range of people internal and external (health, social services, voluntary/private sector staff, service users and carers) relating to the individualised care and risk management plans.

The service model is one of supporting people, whose behaviours challenge services, working closely with specialist practitioners and other health professionals to support this complex group remain in their community setting and, where an appropriate admission has occurred, to be discharged promptly and safely in line with national Transforming Care strategy.

We are looking for a confident individual who will be able to provide forensic support. In return the post holder will receive clinical and managerial supervision, appropriate training and CPD opportunities and an annual personal development review.

Main duties of the job

The post holder will be a key senior member of the Intensive Support Team leading on the forensic element of the service. The Intensive support service will work alongside the established community learning disability team to develop, initiate, lead and manage clinical assessment, staff training and special projects consistent with the aims of the Trust, service plan and service operational policy.

Job description Job responsibilities

Provide effective clinical guidance to the designated Intensive Support Team.

Formulate and implement local clinical and operational practices and procedures as appropriate.

Represent the Service Manager at meetings as required.

Ensure effective deployment of staff and other resources within the Intensive Support Team to deliver the required service and agreed standards.

Participate in the recruitment and selection process in accordance with Trust policy.

Ensure effective induction of new staff in accordance with Trust policy.Ensure exit interviews are offered to staff on leaving the organisation/moving teams within the Trust.

Ensure the implementation, monitoring and updating of an operational policy and philosophy for the clinical team.

Ensure the principles of Clinical Governance are incorporated in service delivery by maintaining a high quality service based on agreed clinical standards.

Ensure adherence and implementation of Effective Care Coordination within their scope of responsibility.

Ensure that Trust policies and procedures are understood and adhered to by all staff within their scope of responsibility.

Develop local protocols involving team members to ensure a high standard of service delivery is achieved and maintained.

Lead and ensure staff participation in the formulation of new ideas/suggestions to improve service user care and service delivery.

Ensure that the Managing Performance Policy is adhered to.

Ensure that the Managing Attendance Policy is adhered to.

Participate in the performance monitoring of services in accordance with national and local targets.

Provide statistical information on workforce and service user activity as required.

Assist in the maintenance and updating of the teams equipment inventories and service schedules.

Ensure regular servicing, maintenance, repair or replacement as required adhering to Health & Safety, COSHH and Medical Devices Agency regulations.

Ensure effective processes of communication are established, maintained and regularly evaluated within designated team.

Establish positive networks with partner agencies to support the development and delivery of intensive support team and Community LD Services.

Maintain and monitor a framework for risk management and Health and Safety that ensures timely and effective planning and appropriate actions.

Undertake Serious Untoward Incidents investigations/reviews as required.

Investigate complaints and incidents in accordance with Trust policy.

Ensure all staff are provided with the necessary clinical and management supervision regarding the co-ordination and delegation of duties, to ensure a comprehensive and consistent delivery of care is maintained.

Ensure that all staff are regularly updated and have knowledge of all emergency procedures.

CLINICAL

Assume a lead in providing effective clinical assessment and interventions to meet the complex needs of people who have a learning disability with forensic behaviour; particularly those whose behaviour presents serious risks to the community and/or brings them into the Criminal Justice System.

To act as an autonomous senior practitioner with continued responsibility for a caseload, providing clinically led interventions to clients with complex challenging behaviour and /or forensic behaviours who may be at risk of placement breakdown and/or hospital admission.

Provide clinical specialist practice in relation to patients with complex needs who may display forensic risk / behaviours, undertaking complex assessments, formulations and risk management plans in conjunction with the patient, their families /carers and other professionals.

To develop PBS intervention plans and risk management plans ensuring the service user, family / carers are able to understand and implement the interventions

Provide training to a wide range of people internal and external (health, social services, voluntary/private sector staff, clients and carers) relating to the individualised care and risk management plans for service users with complex needs.

To provide assessments and reports to a range of health and social care professionals.

To undertake care coordination responsibilities for service users within the CPA / care coordination framework, ensuring coordination an implementation of risk management plans.

To take the lead role in coordinating and chairing reviews / case conferences for individuals with a learning disability and complex care needs.

Work in a professional and collaborative manner with the multi-disciplinary team including joint work with commissioning colleagues in line with national Transforming Care strategy.

Participate and promote positive intra-agency working.

To be involved in the reviewing of placements advising social care colleagues of individualised needs of service users with the most complex needs.

To support the management and analysis of the learning disabilities dynamic support database, with active involvement and management of those individuals whom are amber and red.

To provide an advisory function to staff in inpatient areas in relation to service users with the most complex behaviour when necessary.

To support the coordination of timely and appropriate discharge of complex individuals who have required an inpatient admission.

To take a lead role in responding to crisis situations for people with a learning disability and complex needs on the dynamic support database.

EDUCATION AND DEVELOPMENT

Ensure all junior staff on the intensive support team have an up to date Individual Personal Appraisal and Personal Development Plan in accordance with Trust policy.

Provide clinical supervision to junior members of the enhanced intensive support team

Lead and support the implementation of evidence based practice in line with current research.

Facilitate preceptorship and mentorship processes for all staff including those in training.

Demonstrate the acquisition and application of knowledge in line with current research and evidence-based practice.

Undertake appropriate continuing professional development.

Promote a positive learning environment by contributing to the teaching, training and supervision of junior staff and learners within the organisation.

To provide a consultancy function to external organisations to embed an ethos of positive behavioural support.

To establish, design and deliver individualised training programmes for service users, family members, carers and other professionals in relation to challenging behaviour, PBS, risk management and reasonable adjustments.

Provide evidence to support development around the Trust Appraisal system which may include the KSF or other competency profiles.

Participate in regular supervision, engage in reflective practice and be committed to continued development.

Develop own skills and knowledge and contribute to the development of others.

RESEARCH AND AUDIT

Ensure staff awareness of Clinical and Corporate Governance and the need for research based practice.

Undertake and participate in appropriate audit, quality assurance programmes and research as directed.

To support the clinical lead for the service in collating data analysis and information required in relation to agreed measure for the overall functioning of the intensive support element of the LD service.

To support with audit and research in relation to challenging behaviour and intensive support function.

Person Specification Qualifications Essential
  • Postgraduate qualification i.e. masters or equivalent level of training through experience
  • Up to date Professional Registration (to be maintained) e.g. RMN, RNLD, OT, SW
  • Experience of working with clients with learning disabilities/mental health issues
Knowledge/Experience Essential
  • Direct experience of working with people with a learning disability and complex needs, especially those who display complex behaviours
  • Evidence of experience of working with clients who present forensic behaviours
  • Clinical Experience working with people with a learning disability in a community setting.
  • Evidence of experience of working with clients who present forensic behaviours.
