Homerton Psychological Medicine is the Mental Health Liaison Service for the Homerton University Hospital. We're excited to be offering opportunities for Band 6 Liaison Nurse/Social Worker to join this highly skilled and successful team.

The post holder will be based in the Emergency Department assessing service users in an emergency setting, devising and implementing care, as well as assessing patients on the general acute wards of the Homerton University Hospital. Part of the role will also involve teaching to general adult healthcare staff. The post involves shift work, covering 24/7 on a rota basis.

Main duties of the job

  • To carry out the initial assessments of a person in a mental health crisis within the agreed KPIs
  • To effectively risk assess service users using Risk Assessment frameworks
  • To report all relevant information to the referring agent and keep them informed of the service users progress
  • To assess, plan, set goals with the service user, carers and families; implement and evaluate care using psycho-social interventions and skills to meet individual needs
  • To actively support the person in crisis in self-management of their symptoms through the formation of their care plans
  • To act as crisis coordinator for service users and undertake referrals to other services.

Person Specification Education/ Qualification/ Training Essential
  • oRegistered Mental Health Nurse, Registered Social Worker
  • oCurrent Professional Registration
  • oMentorship Preparation or equivalent
  • oRecognized post registration training
  • oCBT and/or brief intervention /solution focused therapy and /or motivational interviewing or willingness to undertake
  • oRisk assessment and management training or willingness to undertake
Desirable
  • oEvidence of continual professional development
Experience Essential
  • oSubstantial post qualification experience in the field of mental health crisis
  • oPrevious experience at working autonomously
  • oExperience of working in a multidisciplinary mental health team
  • oExperience of working in a multi-cultural environment and of ensuring that nursing practice is culturally sensitive
  • oExtensive and demonstrable experience of assessing, planning, implementing and evaluating programmes of care
  • oEssential and demonstrable experience of writing concise and clear clinical reports
  • oExperience of contributing to the development of new services preferably in mental health
