We are looking for enthusiastic professionals with nursing, social work or allied health professional qualifications with drive and passion to join Buckinghamshire Continuing Healthcare Team. This is a Monday to Friday, county wide service based in High Wycombe.

This is a rewarding and interesting role which will enable you to work in partnership with multidisciplinary teams across Health and Social Care establishing healthcare needs and supporting high quality care for clients and their families in the community.

Main duties of the job

Key aspects of the role include;
  • To complete a comprehensive assessment to determine eligibility for NHS Continuing healthcare.
  • To provide a case management service including annual reviews for eligible clients for an appropriate package of care or placement.

The successful applicant will need to be able to produce large, detailed written reports and therefore you will need to be proficient in the use of Microsoft Word

We are a friendly and welcoming team. You will be allocated a mentor and will be fully supported through your induction using a novice to expert programme. By the end of your first year you will achieved the following key skills;
  • Chairing complex MDT teams
  • Clinical analysis and detailed report writing skills
  • In-depth knowledge of the NHS Continuing Healthcare National Framework and key supporting legalisation such as Mental Capacity Act
  • An understanding of integrated commissioning

I started my HCM role over 3 years ago bringing with me skills from different areas of nursing, both from abroad and UK settings. I find that this profession comes with benefits and challenges at different levels and in diverse ways, depending on the area of the nursing but surely is rewarding in many ways.

This role has proven to have happened at the right time to help me balance my personal and work life. In my experience, getting quality time with my family and friends by not having to work nights, shifts, weekends or bank holidays contributes a great deal to being a better clinician. The role is complex and sometimes can be very stressful, but I find it also extremely rewarding when seeing what a difference the service I am part of makes in our patients and their loved ones lives.

Managing caseloads can be challenging/tricky at times but offers support and continuity for our patients which is the actual aim of this role. I would definitely not be able to bring my contribution without a fabulous team alongside me and the greatest work environment I have ever been in. I am fortunate enough to be part of Oxford Health - Continuing Care lead by lovely people and best professionals.

You will need to be a car driver as assessments can take place in the Acute sector, clients homes, Community Hospitals and Hospices across Buckinghamshire and out of area where required. If necessary, adjustments can be considered in line with the Equality Act 2010. There is an opportunity to work flexibly to meet the needs of the service and this includes some working from home.
