Health in Mind - Assistant Primary Care Mental Health Practitioner AfC Band 5
We are pleased to be able to offer a new and exciting opportunity to join the team at Health in Mind. The person that is successful in applying for this post will be responsible for providing rapid and responsive assessment, care-planning, signposting and brief treatment to individuals referred to the service by a number of routes, including self-referrals and by General Practitioners. There are Routine 20-day and Priority 5-day referrals to manage effectively. The Assistant Primary Care Mental Health Practitioner (PCMHP) role ensures that people referred to Health in Mind are quickly and efficiently able to access the most appropriate treatment for their mental health difficulty.
Main duties of the job
Health in Mind is a large primary care mental health service that serves the people of East Sussex, with the exception of Brighton and Hove. Due to recent developments within the service, we can introduce this new vacancy in the PCMHP team. Health in Mind covers three localities and it is possible currently that a new PCMHP could work in any of those areas, which are Hastings and Rural Rother, Eastbourne Hailsham and Seaford or High Weald Lewes and Havens. PCMHPs are responsible for a defined GP surgery or group of GP surgeries in a specific location within the locality. Although you will be working primarily in one geographically defined area you may need to work outside of this area in meeting the overall demands of the service. This could be offering clinical work in other areas, attending meetings or visiting stakeholders across the locality.

Person Specification Qualifications Essential
- Professional registration with a minimum of 12 months post qualification experience in either a Community or Inpatient mental health setting
- Knowledge of MHA (1983/2007).
- Knowledge of Mental Capacity Act (2005), to include practical knowledge of FACE documentation, DOLS and Best Interest decisions.
- Child and Adult safe guarding procedures
- Change management at organising and prioritising their workload
- Demonstrate ability to enhance care through evidence based knowledge.
- Demonstrate professional development relevant to the specific care group
