Senior Community Mental Health Nurse (Band 6)

Community Rehabilitation Pathway

Shropshire, Telford & Wrekin

37.5 hrs per week (full time)

An exciting opportunity has arisen for an experienced and dynamic Band 6 Community Mental Health Nurse to work within the new and developing Community Mental Health Rehabilitation Pathway across Shropshire, Telford & Wrekin, working towards the below agreed values and vision for people with complex mental health needs who often experience multiple and interconnected challenges so require a service that provides an intensive recovery and rehabilitation approach.

That everyone has belief in me and works alongside me to live the best life I can

Is centred on my needs -not centred on my diagnosis, instilling hope, and balancing risk to maximise the best outcomes for me and my independence.

Is built on my strengths -supports my aspirations, interests and goals and recognise that these may change over time, identifying changes in my needs early to ensure timely interventions and seamless transitions

That offers proactive support- those who support me are proactive, consistent, flexible and transparent, seeking feedback and aiming to continually improve

That works together -so that those who are important to me are recognised and included, and seen as a critical part of the team around me

Local access -provided locally, not impaired by organisational boundaries and promotes collaborative working

The service, which is in its early stages of development, will work with people at an early stage of their recovery, where previously they would have been supported in an inpatient environment for an extended period of time. The service will work with people in a least restrictive way and collaborate closely with the individual, their support networks, and other professionals to offer just enough support, so that people can take the risks they need to in order to reach their potential, and live the best life they can. The service will work with people who have experienced trauma in their lives and can present with behaviours that can challenge. Therefore, there will be a need for the service to work hard to build helpful and trusting relationships, offering stability and consistency.

What we are looking for:

We are looking to recruit a forward thinking and dynamic senior community mental health nurse with good leadership skills, relevant knowledge and experience, and an ability to demonstrate resilience and confidence in managing complex risk.

We are looking for someone who possesses excellent communication and interpersonal skills, effective clinical reasoning, and an ability to build positive multidisciplinary team relationships, and work collaboratively with external agencies, patients and their families/carers.

The post holder needs to be able to demonstrate drive and motivation in providing recovery-focused and evidence based interventions to individuals with complex severe and enduring mental health disorders.

The post will involve a responsibility for your own caseload of complex service users for care coordination within a Care Programme Approach. The environment can be fast paced and challenging, however, highly rewarding.

The post holder would have the opportunity to use their professional skills to contribute to the development of services and develop themselves personally and professionally in order to provide an evidenced based service.

Being involved at the beginning of this new service in Shropshire, Telford and Wrekin is such a fantastic opportunity.

What we can offer you:
  • Peer support
  • Regular supervision from within the leadership structure of the service
  • Professional development opportunities and training
  • Personal development reviews
  • Promote innovations locally and nationally.
  • Evidence base, outcome measures, audit.

The initial hours of work will be Monday to Friday 9am - 5pm but you will be expected to be flexible within your working approach and delivery. As the service develops we will move to a the provision of a 7 day a week service working 8am-8pm.

The candidate must be able to meet the travel requirements of the role in order to travel across the full geography of the catchment area.
