The Team consist of experienced multi disciplinary colleagues, including Nurses, Social Workers, Psychiatrists, O/T and Psychologist all working to a Recovery based approach. You will have ongoing supervision with opportunities to develop via relevant training, appropriate to this role.

The role within the team will allow you to develop skills in both assessment and risk management.

Main duties of the job

An exciting opportunity has arisen for a band 6 Mental Health Practitioner to join the Home Based Treatment Team in Trafford. This is a fantastic opportunity to join an enthusiastic and dynamic community team. Home Based Treatment Team provides an ageless service to people who live in Trafford. We provide an alternative to inpatient admission and support people with severe and acute mental health problems in their home whilst undergoing intensive treatment for their mental health needs.Candidates will be expected to work as a keyworker providing assessments and inform treatment plans for those people under Home Based Treatment. You will be a good communicator and self-motivator who delivers high quality care. You will be expected to carry out in depth assessments in the community and you will be expected to contribute and formulate care plans and risk management plans.

Job description Job responsibilities

The post holder is required to offer interventions to those suffering with mental health problems that are in an acute crisis. To undertake bed gatekeeping assessments when an admission to hospital is requested. The post holder is required to offer supported discharge assessments of current in-patients.

The postholder will maintain and review relevant documentation to a high standard and reviewed within operational time scales, in line with relevant clinical pathways as outlined within the HBTT operational procedure.

The post holder will be expected to work shifts, that include weekends, early, late and night shifts and on call (over night).
