We are looking for an experienced RMN/ LD/ RGN nurse to take on the role of Staff nurse at our Dementia Assessment Unit. The DAU is a mixed sex ward with a Gold accreditation for a dementia enabling environment. It is part of the older peoples dementia care pathway. The unit assesses patient needs and creates a treatment plan to enable patients to move on to the appropriate community setting to support their care needs closer to home.

The post holder will be responsible for helping the co-ordination and administering assessments of patient care needs, the development of treatment programmes and the implementation and evaluation of these within a multidisciplinary team. The post holder will support the development of the team to ensure in depth holistic assessments are completed to include assessment relating to mental health, physical health and social needs. The post holder will also be flexible to support the Older Peoples Dementia care pathway in supporting preventable admissions to hospital and facilitating timely discharges.

Main duties of the job

To provide and coordinate a high standard of care to the patients cared for on the Dementia Assessment Unit.

Act as a lead practitioner to ensure that the care of patients is of the highest quality.

Supporting discharge planning from admission.

Delegate work to staff and be the responsible clinician that ensures carerequirements are implemented satisfactorily.

Distribute medication in line with Trust policy and NMC guidelines.

Maintain administration information systems as required.

To comply with the correct implementation of the Mental Health Act.

Main duties:To work as a Staff nurse ensuring that the assessment, planning,implementation and evaluation of care for a designated client group is carried out to the highest standard.To manage a caseload and delegate appropriately to clinical practitioners and assistant practitioners.Assists the Team Manager and Assistant Ward Managers in ensuring that agreed standards of professional activities within the unit are achieved.Formulates and influences new ways of working within the service. Supporting planning for discharge upon admission and working with the wider clinical network that supports this process.Support the Team Manager and Assistant Ward Managers in the staffing co-ordination/deployment on the shift.Assist the Ward Manager and Assistant Ward Managers in ensuring that effective communications are maintained with other professionals, disciplines and teams.Guide/mentor/supervise supporting staff as required facilitate/deliver ongoing or functionally relevant activities in the most appropriate setting.Evaluate treatment outcomes and meet emerging needs. Referring on to address unmet needs as appropriate and necessary.Acts as a role model to staff; demonstrating compassionate dementia care whilst sharing your own dementia knowledge.Ensure that patient records are maintained and that information is passed on in a co-ordinated manner.To participate in health studies and research into new patterns/methods of care to further enhance set standards of care. Supported by senior members of the clinical team.Ensures that Trust Policies, Protocols, Procedures and Guidelines and Professional Codes of Conducts are implemented and maintained. Suggest any policy revisions to the Team Manager.Undertakes in house and external auditing, monitoring quality and maintaining high standards expected.Working as part of a TeamGives professional advice and clinical support to staff within the service and refers to the Team Manager or other professionals when necessary.In conjunction with the Team Manager and Assistant Ward Managers identify staff development needs.Managing SelfParticipate in regular supervision.Attend all mandatory training.Participate annually identifying, developing and agreeing your own development plan with your line manager using the Trust appraisal and KSF process.Comply with all Trust policies, procedures and protocols.Pay regard to materials and equipment.Carry out duties with due regard to the Trusts Equal Opportunity Policy.Seek advice and support from Team Leader whenever necessary.
