Main duties of the job

We are looking for dedicated and pro-active nurses or registered practitioners to work as part of our well established Street Triage Team. The team will work closely with the Cleveland Constabulary but will be aligned to the wider crisis services already provided by Tees, Esk and Wear Valleys NHS Foundation Trust within the locality.

The team will work in partnership with Cleveland Constabulary to identify people with mental health needs and support them into appropriate services, creating effective partnerships and tackling stigma. You will triage and assess calls made to the Police Force Control room alongside offering face to face assessments to people with mental health problems, when they come into contact with the police. We are looking for practitioners who can promote a culture of evidence based practice and provide excellent, responsive person centered care.

Person Specification Qualifications Essential
  • Current professional registration with a recognised professional body
  • Evidence of Continuing Professional Development
Desirable
  • Leadership or management qualification
  • Post-graduate qualification in a relevant area
Experience Essential
  • Significant experience working with people with complex mental ill health, behavioural and social issues, in a community setting
  • Experience in assessing and treating mental health patients in crisis.
  • Working collaboratively with service users and their families/carers.
Desirable
  • Leadership or management experience
Knowledge Essential
  • Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act).
  • Detailed understanding of Safeguarding and its application in practice.
  • Clinical Risk Assessment and Management and its application in practice.
