Having successfully recruited to several Primary Care Network (PCN) areas, we are now seeking to appoint Clinical/Managerial Lead (Clinical/Counselling Psychologist, Occupational Therapist or Mental Health Nurse) with the necessary skills to fill the position in Runnymede & Spelthorne area covering either 2 PCNs (Part-Time role, 22.5 Hrs / 3 Days per week) OR 3 PCNs (Full-Time role, 37.5 Hrs / 5 Days per week).

We have an exciting opportunity to shape the services of the future. The General Practice Integrated Mental Health Services (GPimhs), working with a wide range of stakeholders, provides support for people with significant mental illness closer to their communities and homes. Staff from NHS, social care and the voluntary sector work closely with GPs to improve local services for thousands of people.

Mental health specialist providers Surrey and Borders Partnership NHS Foundation Trust and Berkshire Healthcare NHS Foundation Trust are early adoption sites for this transformed model of care in primary care networks. Primary Care Networks, each with a population of30,000 60,000, host a team of mental health professionals including NHS, voluntary sector and social care roles. The GPimhs team offer extended appointments, plus access to therapies, physical health checks and pharmacists, all accessed via their local GP practice.

Consideration will be given to both full-time and part-time applicants.

Main duties of the job

We need you to help us continue to develop and shape a major new and innovative approach to providing mental health care within the community across these areas to transform how services support the health and wellbeing of people with complex mental health conditions.

As a Clinical Lead, you will manage two to three primary care mental health teams within defined geographical locations, working closely with GP and Surrey and Borders colleagues. You will also see more complex primary care presentations, and those with serious mental illness discharged from secondary care.

The candidate must have a full and valid driving licence and access to a vehicle for work purposes due to the community element of this role.

Job Overview

To act as Clinical Team Leader for developing and delivering Mental Health services within a new Primary Care Network model and ensure the service operates in a safe, effective & efficient way as part of the whole system of mental health services. To provide clinical leadership and provision of evidence based biopsychosocial practices to people frequently presenting in primary care to ensure they have an active role in identifying, planning, and meeting their individual needs.

Suitable for someone who is passionate about ensuring a high-quality holistic approach to the care and support provided to a cohort of people who have been identified as in need of mental health support in a primary care setting.

Key Responsibilities

To provide Clinical Leadership, managerial responsibility and oversight of the new multi-disciplinary mental health team within the identified PCN sites.

Responsibility for the continued development of the Primary Care MH Team within the PCNs, including identification and embedding of evidencebased Biopsychosocial models of care, informing assessment, formulation and interventions of the team.

Responsibility for recruitment and development of staff structure, pathways and service specifications.

To work closely with GPs, the primary care team and partner organisations of the PCN mental health service in establishing the new model of care and embedding the service delivery model to deliver to the identified patient cohorts.

To provide MH advice and guidance to other PCN-MH team members and specific advice and guidance to GPs regarding complex mental health presentations in primary care.

To provide specialist screening and assessments of complex patients who frequently attend their General Practitioner, based upon the appropriate use, interpretation and integration of complex data from a variety of sources including a range of biopsychosocial tests, self-report measures, rating scales, direct and indirect structured observations and semi-structured interviews with patients, family members and others involved in the patients care.

To formulate, implement and review plans for the formal treatment and/or management of a patients psychological and occupational problems, based upon an appropriate conceptual framework of the patients problems, and employing methods based upon evidence of efficacy, across the full range of care settings.

To be responsible for implementing a range of biopsychosocial interventions for people, carers, families and groups, within the Primary Care Network sites, adjusting and refining formulations drawing upon different explanatory models and maintaining a number of provisional hypotheses.

To develop and maintain good communication and relationships with the wider mental health services to develop seamless pathways between primary and secondary care that ensure people receive the right service at the right time in the right place.

To evaluate and make decisions about treatment options considering both theoretical and therapeutic models and highly complex factors concerning historical and developmental processes that have shaped the patient, family or group.

To undertake risk assessment and risk management for individual patients and to provide advice to other professions on complex risk presentations and risk management, in line with Trust and inter-agency policies and procedures with the aim of overall risk reduction.

To provide supervision to other MDT staffs Biopsychosocial work, as appropriate, contributing to appraisals and addressing performance management issues as and when needed.

To contribute to the service evaluation of the new model of care including the development, evaluation and monitoring of the teams performance (against defined key performance indicators), operational policies and services, through the deployment of professional skills in research, service evaluation and audit. To advise both service and professional management on those aspects of the service where psychological and/or organisational matters need addressing

To ensure that the needs of people with protected characteristics are addressed within local services and future service developments

Person Specification Experience Essential

• Significant experience of operating effectively as a senior leader in a health or social care environment.
• Proven ability to successfully manage Change and Service Redesign.
• Experience of providing clinical supervision and mentoring Experience of delivering educational materials to services and partner organisations.
• Demonstrates evidence of highly effective clinical reasoning skills.
• Experience of representing services at Trust and with partnership organisations.
• Experience of facilitating biopsychosocial formulations within an MDT setting.
• Experience of working with people with complex needs.
• Experience of using highly effective communication skills across systems.

Qualifications Essential

• Applied Psychologist / Occupational Therapist / Mental Health Nurse.
• Current Registration with the Health Care Professions Council (HCPC) or RMN Post Qualification training /and experience in personality disorder and complex trauma Evidence of continuing professional development.
