We are currently transforming Mental Health Services in Somerset and have an exciting opportunity for you to be part of an expanding Community Mental Health Service. We are seeking a Mental Health Practitioner (either as a Mental Health Nurse, Occupational Therapist or Social Worker) to join our Community Mental Health Service in Taunton.

This is an exciting time to join a thriving and innovative Mental Health Trust that has received significant investment for mental health services in Somerset and is a trailblazer site for the Community Mental Health Framework. This enables us to offer an excellent service to people with mental health difficulties ensuring that people can have the right help at the right time, in the right place, operating a no 'wrong door' ethos.

We have recently been recognised for this dynamic way of working and have been awarded the 'Collaborating for Improvement National Improvement Award' from the NHS Big Conversation for Improvement as well as Mental Health Trust of the Year.

Main duties of the job

The post holder will deliver a high quality, comprehensive mental health service for patients with severe and enduring mental health difficulties within the Community Mental Health Service. The role involves managing a defined caseload, with a focus on working with individuals who experience a complex range of needs and difficulties, and supporting their significant others.

Acting as a key member of the multi-disciplinary team, the post holder will provide assessment, psychosocial, evidence-based interventions using a collaborative, recovery approach and will beskilled at positive risk management. The post holder will contribute to the clinical leadership of the team, providing supervision for other team members, as agreed with the Team Manager, and participate in team and service development and evaluation. Liaison with a wide range of agencies is central to the role.

The post holder must hold a full driving licence and have permanent access to own transport and have appropriate insurance.
