A specialist opportunity has arisen for an Urgent Care Nurse with a primary responsibility for Older Peoples Mental Health Liaison in Hastings. The post has arisen due to the current post holder relocating to another part of the world. This is a really exciting time to join the team as we are continuing to develop the core 24 model, to provide an enhanced Urgent Care Service that operates over 24 hours.

As part of this we have The Haven (formerly known as the Urgent Care Lounge. The Haven provides a safe, calm and therapeutic space which can provide an alternative, calm, safe space to the A&E department and avoid long waiting times for the patients we provide a service to.The Older Peoples mental Health Service receives referrals from the Conquest hospital wards for people who have become mentally unwell whilst receiving care for a physical illness. or are experiencing a relapse of a previously diagnosed mental health condition. The A&E department also refer , and the Irvine rehabilitation unit in Bexhill.

Main duties of the job

The post holder will provide mental health assessment, treatment, signposting and onward referral for patients receiving physical health care in the general hospital environment in East Sussex. They will also provide education, training and information on mental health for general hospital staff working in A & E, medical & surgical assessment units and wards.

We are looking for an individual with an excellent working knowledge and understanding of Older Peoples Mental Health and the correlation between Physical and Mental Health conditions. Excellent communication skills are essential and someone who is able to work autonomously and at pace within a wider multi-disciplinary team.

Experience of working within Mental Health Liaison or a similar clinical environment would also be an advantage. In return, we can offer you training and development, variety, a rewarding challenge and the opportunity to work within a friendly and supportive Liaison service.

About us

This is a permanent and full-time post (37.5 hours per week), however, alternative working arrangements such as job shares will be considered on an individual basis. The post-holder should be willing to work flexible hours as required to support delivery of the 24/7 Urgent Care Pathway for Mental Health. The Liaison team is based in the St Anne's Centre on the Conquest Hospital site in Hastings, although some travel across sites in East Sussex may be required.Hastings is recognised as an upcoming coastal town with a vibrant cultural scene, providing affordable housing, excellent transport links and a range of outdoor activities in the historic 1066 countryside.

This post offers the Trust's £2000 Golden Hello for band 5 or 6 Nurses, or a relocation package up to a maximum of £2000 (subject to terms and conditions).

Job description Job responsibilities

As a Mental Health Liaison Practitioner, you will operate as an autonomous practitioner, undertaking comprehensive assessment and formulation, including risk assessment that leads to an agreed plan. You may provide a range of interventions including signposting, support, psychosocial interventions, therapeutic interview, and brief psychotherapeutic interventions. An integral part of this role includes effective liaison with other parts of the health system, including primary care, acute and secondary care teams, as well as social services, emergency services and non-statutory agencies. You will work closely with the acute hospital, acting as a single point of contact of referral and serve the agreed population of 65 plus. There will also be opportunities in teaching, education and assisting non-mental health staff to respond to mental health needs more effectively.

Person Specification Qualifications Essential
  • Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role
  • Completion of Mentorship Course/ENB equivalent
  • Evidence of continuous professional development
Knowledge/Experience Essential
  • Significant post qualifying experience in the relevant care group
  • Experience of working alongside people with complex health problems in the designated care group
  • Experience of supervising staff
  • Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
  • Experience of working consultatively with professionals
  • Experience of conducting clinical assessments including risk assessments
  • Experience of partnership working within and across various statutory and non statutory teams and agencies
  • Experience of working within the CPA process
  • Understanding of clinical governance
  • Previous experience of working in the community
