An exciting opportunity has arisen for an enthusiastic, committed and passionate band 6 community nurse. The post is based within the haringey older people's community mental health. The post holder will work with the older people's service carrying out specific assessment and interventions. They will manage a case of patients with enduring organic and functional mental health problems working alongside a wide range of clinical teams. Successful candidates will have strong engagement and motivation skill and be able to work well with service users with complex mental health needs.
Main duties of the job
Ensure that the service of the CMHT i.e. nursing, social care, occupational therapy and psychology is carried out effectively within a multidisciplinary framework and in collaboration with other agencies. Maintain contact with service users on their caseload during periods of admission, including attendance at MDT meetings.
To take lead in the planning and implementation of the client's reintegration back into the community.
To provide effective and timely care within a multidisciplinary team setting for Older People who present with functional and/or organic mental health problems.
To assess care needs, devise care programmes including risk assessment and implement and evaluate these in collaboration with individual service users and also their carers; where its appropriate.
To support and mentor less experienced staff and teach students from related professional backgrounds who are on placement with the CMHT.
To undertake duties designed to extend clinical and managerial expertise as delegated by the Deputy CMHT manager when required. This will include deputising in the absence of the Principle Practitioner.
To provide a full care management service to service users and their carer's.
