EPUT is currently undertaking a substantial transformation project within our community mental health services, something of which supports the NHS 5 Year forward plan and the increase in mental health funding announced by the Government.

This post is advertised as full time, but there is an opportunity for you to work these hours over 4 or 5 days per week, giving you more flexibility and work life balance.

Due to these exciting changes we are looking for a Primary Care Mental Health Practitioner (PCMHP) to join the Adult Community Mental Health Services, Mid Essex. This is an exciting role for a Primary Care Mental Health Practitioner to support the way forward in the delivery of Community Mental Health Services

Main duties of the job

  • Provide a seamless service in order to aid the Patient's recovery and Well-being plans in the community.

o Work in conjunction with the GP, Primary Care team and Consultant Psychiatrists to ensure that patients are supported in the most appropriate clinical practice environment from a Multi-disciplinary perspective.

o Ensure safe Clinical Practice takes place, maintaining Care Quality Commission standards at all times and in line with the NHS Constitution.

o Triage patients and facilitate them receiving the most appropriate support for their needs ensuring people are not passed around the system.

o Deliver effective and safe services which support the key elements of Clinical/Practice governance.

o As the Primary Care model develops, the post holder will focus on supporting the team's continuous improvement in the quality of their service, whilst safeguarding and maintaining a high standard of care by creating an environment in which excellence will flourish.

o Provide and undertake detailed Physical Health investigations such BMI, Blood Pressure and request Blood Test as appropriate.

Job description Job responsibilities

The successful candidate will assist with undertaking physical health checks for patients on the SMI register and support patients who are discharged from secondary care. This innovative approach allows for new roles to complement the existing traditional General Practice roles to manage more patients at the point of contact. The teams skill mix has been designed based on a local population needs assessment and will consist of Physiotherapists, Clinical Pharmacists, Advanced Nurse Practitioners, Physicians Assistants, Community Psychiatric Nurses and Paramedics.

They will be highly skilled and a motivated Mental Health Nurse and be part of the Enhanced Primary Care team, at the forefront of establishing this innovative development in Primary Care Mental Health Services. The postholder will work with service users and carers, close to their home, to deliver highly accessible and responsive interventions via flexible referral routes. The key principles of the team are to offer interventions within Primary Care to promote a truly integrated, holistic approach to care.

Whilst driving is essential due to the demographic area of Mid Essex, where someone doesnt hold a full driving licence, applications will still be considered for those wishing to travel by public transport.

The post holder will be able to demonstrate the following:
  • Registered Mental Health (RMN)
  • Evidence of Continued Professional Development (CPD)
  • Experience of working within a Mental Health setting
  • Good oral communication skills based on fluency on the English Language
  • Ability to manage difficult situations sensitively by listening, showing tolerance and remaining objective

Annual leave entitlement

In line with the Agenda for Change terms and conditions, on appointment to the NHS you will be entitled to 27 days annual leave plus Bank holidays. Your entitlement will increase after 5 years NHS Service to 29 days plus bank holidays per year and then increase again to 33 days plus bank holidays after 10 years service.

Relocation Expenses

The Trust can offer a generous relocation package of up to £8000, which is dependent on eligibility and offered under specific terms.

Chelmsford

Chelmsford is a busy bustling town which consists of a shopping centre, restaurants and bars. It as easy transport links to central London which is 30 miles away and is also easily accessible by car as the A12 is the main road from London into Chelmsford and also allowing easy access to theM25and the rest of the motorway network. Chelmsford has been redeveloped in the last few years providing new homes and attracting new residents to the area. Maldon and Braintree are close by and are easily accessible by car. Theyre smaller towns, but still have the essential shops and restaurants for all of the residents in the area.

  • Experience of working within a Mental Health setting
