We are excited to have a vacancy for a Senior Occupational Therapist to provide therapy support in our team based in Peterborough Intermediate Care Team in the community within peoples homes, but will be covering Hunts locality as the need arises as part of the wider North Intermediate Care Service.

Weprovide outstanding, patient focused, community based health care and therapy, optimising peoples potential to achieve their goals by supporting earlier discharge from hospital; early assessment for care needs and avoidance of hospital admission.

Main duties of the job

  • To work as an autonomous practitioner within the integrated multi-professional team, neighbourhood teams.
  • To be responsible for own caseload and undertake assessment of patients with complex and multiple pathologies using specialist clinical reasoning skills
  • To identify patient needs, agree goals and provide appropriate therapy interventions, provide adaptive equipment, rehabilitation programmes, and make recommendations for minor and major adaptations in line with current legislation.
  • To keep the patient at the centre of care, ensuring patient and carer participation in decision making.
  • To provide support and education to peers, new staff non-registered staff and students.
  • To supervise junior staff and students, overseeing patient intervention and ensure that Assistant Practitioners and Integrated Care Workers deliver high quality patient care.

To be professionally and legally accountable for all aspects for your own and delegated work.

To work within codes of practice and professional guidelines.

To undertake holistic assessment (including those with complex presentations and multi pathologies) making use of specialist clinical reasoning skills.

To use evidence-based practice, to ensure clinical care is effective and appropriate.

To promote independence and wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure those patients receive the appropriate care in the most appropriate setting.

Organise and manage own time, delegating work appropriately.

Conduct risk assessments and health and safety assessments and includingimmediately reporting any changes/newly identified risks.

To demonstrate the importance of gaining patient and carer consent in all interventions.

To effectively communicate verbally with patients, carers and colleagues using tact and persuasive skills. This may involve using skills where patients have difficulties in communication. Eg hearing loss, diminished sight, depression, speech problems, cognitive impairment, behavioural problems and pain.

To actively manage positive relationships with other agencies involved in the patients pathway.

Actively involved in the development of the service/organisational aims and objectives and the new model of care.

To work effectively within the structure and liaise appropriately with members of the MDT and other agencies by attending MDT meetings, case reviews.

To ensure that accurate and timely written records are kept which comply with the Trust policy and HCPC guidance, reporting on any issues as appropriate.

To provide specialist advice to all members of the multi-disciplinary team, service users and appropriate others, of changes involving current care plans, patients progress and other relevant matters that pertain to the care of the patient.

To work within Trust and COT guidelines and to have a good working knowledge of national and local standards of practice in line with legal and operational requirements.

To demonstrate a good understanding of Social Service and Health legislation.

To support with day to day planning and delivery of a high quality care.

Person Specification Education/Qualifications Essential
  • Degree or equivalent Current HCPC Registration
  • Current HCPC registration
  • Documented evidence of own continuing professional development
  • Willingness to undertake Fieldwork/Practice Educator qualification
  • Member of COT
  • Fieldwork/Practice Educator qualification
Experience Essential
  • Relevant post-graduate clinical experience in a variety of settings
  • Experience and knowledge of housing legislation, minor and major adaptations or willingness to train in this area
