Registered Mental Health Nurse looking for something a bit different?

Are you looking for a role where you can help shape the service and transform experiences in Adult Mental Health for your local population?

Are you an excellent relationship builder who has the best patient outcomes at the centre of everything that you do?

Would you like to enjoy coming to work each day and be part of an amazing team who love their job?

If so then we would love to talk to you!!

Across Hampshire and Isle of Wight (HIOW) we have launched our plan to transform experiences in adult mental health in a programme called ‘No Wrong Door’. This is a new and innovative service led by the local NHS, councils, Public Health, GPs, voluntary and community partners and people with lived experience of mental illness.

Through our research adults with a serious mental illness have told us first contact is the most important point in successfully starting their recovery. When people with a serious mental illness are at their most anxious, they face too many choices with little support.

To tackle this, we are going to:
  • Ensure that at the start of a person's recovery, wherever and however a person accesses care they are met with compassion, understanding, and supported with both their physical and mental wellbeing.
  • Build teams around communities that are close to people’s homes, by working with our partners including the voluntary sector, making sure people’s recovery is close to their personal support networks.
  • Provide people who are working across these physical and mental health services with the right support and training, making sure we have creative career options across the area.

  • Southern Health NHS Trust will be working with Primary Care Networks, known as PCNs, to develop teams of health and care professionals including wellbeing practitioners, nurses, experts in psychology and psychiatry, social workers, and education and employment support workers.

Southern Health NHS Trust’s Portsmouth and South East Division and both the Swan and East Hampshire PCN’s are now in position to offer these exciting opportunities for innovative, flexible, professionals who are able to network and be part of the building blocks for the transformation of frontline mental health services.

    East Hampshire PCN

It has an extensive geography and covers 48000 patients. The GP practices are: Badgerswood and Forest, Pinehill, Riverside, The Grange surgery, Clanfield, Horndean & Rowlands Castle. We have a well-structured social prescribing team in place covering the area in three teams – north, central and south. Also, there is a wellbeing coach in the form of a mental health practitioner for children and young people as part of that so the addition of a role for someone to help with the adult population would complete the all age offering and would allow transition from the young person’s team into adults. We have a group of practices that get on well and are happy for the PCN staff to contact them, come into the surgery and generally feel part of the team. Clinical Supervision is a regular session with the clinical lead for mental health and that person is also a conduit to other practices if needed. The team structure also means that this person would not be working in isolation and referrals would come to them via an MDT discussion involving all PCN staff.

This role is about working with people, supporting them at home and in their communities.

    Swan Medical Group PCN

The Swan Medical Group is one practice but consists of four GP surgeries; The Swan Surgery in Petersfield, Liphook Village Surgery and Liphook & Liss surgeries in Hampshire.

This is a new and exciting opportunity for an experienced individual to join our already established team of Social Prescribers and to help develop our Health and Wellbeing team. The post holder will work across all sites and regular Clinical Supervision sessions would be in place with the clinical lead to ensure support and a full MDT team approach.

We are a supportive and forward-thinking practice with an excellent clinical team. We are also a training practice for doctors and are supportive of personal development & ongoing training for all staff.

Swan Medical Group is proud to have achieved an ‘Outstanding’ rating in our recent CQC inspection and we aspire to be a local leader in delivering exceptional integrated healthcare. If you are keen to develop your career and work at an outstanding level, we would love to hear from you.

Employed by Southern Health NHS Trust and funded jointly these roles will be based within PCNs to develop relationships between organisations and influence the transformation to provide the right services for our population by the right people at the right time.
