Are you committed to improving outcomes for children and young people with Autism and/or Learning Disabilities . Do you want to be part of a dynamic project aimed at reducing the risk of this group of young people going into crisis . Do you want to be the person that sits at a cetr meeting asking how we can be creative with designing bespoke packages of care for this patient group . Do you believe that children young people and carers should be at the centre of co-producing services. Do you want to be part of a new 8-8 service so young people with Autism can access appropriate and equitable care.

Enhanced support nurse offering support , liaison , interventions and training for the network around a young person with Autism and or Learning disability. Operating an 8-8 service .

Main duties of the job

Organising care intervention and aiming to reduce the risk of admission. Utilising the principles outplayed in the NHS Long term plan and the Transforming care agenda to offer better outcomes

Supporting the pilot scheme to educate the network around children and young people. To undertake assessments , support admission avoidance packages, to support children on the dynamic support register
