The Patient Flow Hub is a Trust wide service which provides bed management services for adults and older people's services within Pennine Care NHS Foundation Trust. The service covers the geographical areas of Stockport, Tameside, Rochdale, Oldham and Bury.

The main hub is based at Fairfield General Hospital but the post holder will be expected to operate from other sites as service needs require. The Patient Flow Hub, although a primary bed management service, will have a key discharge coordination function within it and the post holder will be expected to lead on discharge coordination and support the ward mangers with effective and safe discharges.

Main duties of the job

  • Participate and assist in establishing an effective system for bed management of beds to ensure efficient patient capacity and flow.
  • Oversee all requests for emergency admissions, discussing alternatives to admissions where appropriate to ensure timely response.
  • Minimise incidence of bed crises, using knowledge of impending discharges and extended periods of leave.
  • Actively be involved in the delayed discharge process and assisting the MDT to facilitate timely discharge and escalate cases as appropriate.
  • Contribute to the development pf systems and processes that will support live bed states. Promote and foster supportive relationships with key stakeholders.
  • Develop and maintain processes for effective bed management which will include accessing funding and placement for securing non-NHS beds.
  • Proactive in ascertaining the Trust's position regarding bed management resources.
  • Liaison with appropriate teams and services in the management of interface issues.
  • Immediate problem solving and decision making, to enable both short and long term planning within the Bed Management service.
  • Oversee and monitor the recording of non-contracted activity.
  • Ensure accurate and timely recording of bed management information.
  • Support bed management and patient flow.

  • Ensure bed states are maintained and handovers are completed to the Night Managers.
  • Attend MDT meeting, risk panel meetings, complex case meeting a liaise with CCGs around placements and supporting the coordination of multi-disciplinary team reviews for complex DTOCs.
  • Ensure adherence to bed management protocol.
  • Maintain and demonstrate up to date knowledge, ensuring any gaps in clinical competency are addressed whilst also staying compliant with all business policies and procedures.
  • Prioritising repatriations in accordance with policy based on clinical risk and priorities across the Trust.
  • Be a resource to junior members of staff.
  • Deputise in the absence of the Clinical lead, including authorisation of leave and any budgetary requirements.
  • Support with system wide response to OPEL scores.
  • Ensure recommendations from national policy and guidance are communicated and understood and implemented.
  • Ensure staff attend relevant training and are compliant with mandatory training requirements.
  • Will be responsible for the line management of the Patient flow coordinators.
  • Participate in recruitment and retention for appropriate staff.
  • Support investigations into complaints and provide necessary written reports on behalf of the Clinical Lead.


Person Specification Education Essential
  • Professional knowledge from a post graduate qualification and / or equivalent experience.
  • RMN or equivalent experience.
  • Evidence of ongoing CPD.
Desirable
  • Degree - or working towards completion.
Experience Essential
  • Experience of working as a qualified practitioner and evidence of appropriate clinical management experience.
  • Solid experience in working with individuals who present to mental health services in crisis and have symptoms of mental illness within acute mental health settings
  • Experience of working in multi-disciplinary teams
Desirable
  • Have wide experience in all aspects of Mental Health Nursing.
  • Involvement in operational policy development
  • Involvement in performance reviews for all staff and formulating full clinical are training profile
  • Organisational, managerial and motivational ability.
Knowledge Essential
  • Ability to act as mentor, counsellor, educator and advocate to patients, staffs and relatives.
  • Acute hospital service provision
  • Admissions, transfers and discharge procedures and implementations.
  • Knowledge of risk assessment / management processes.
  • Knowledge and understanding of current mental health legislation
  • Understanding of confidentiality issues and data protection issues
  • Comprehensive understanding of the Discharge Standards, CCG Liaison, Specialist Services Liaison
  • Understanding of the principles of capacity management and bed management procedures.
  • Auditing / data collection
  • Research skills and ability to evidence current / proposal practice
  • Understanding of funding process for placements
  • Working knowledge of Delayed Discharge Criteria and Barriers to Discharge. Understanding of S117 process
Skills and abilities Essential
  • Comprehensive mental health and risk assessment skills
  • Excellent communication skills, both written and verbal.
  • Good analytical skills
  • Ability to demonstrate working knowledge of the Mental Health Act and CPA process.
  • Comprehensive and proficient IT skills including NHS computer systems
  • Ability to form productive working relationships with service users, carers, multi-disciplinary staff and all relevant stakeholders.
  • An effective communicator with the wards, consultants and service managers
  • Ability to delegate appropriately.
  • Ability to manage budgets and resources
  • Ability to induct any staff member to the area with confidence.
  • To establish and maintain close working links with Social Services, CMHT and other secondary care providers.
  • To liaise with other Boroughs regarding patient transfer and be proactive in the transfer of these clients.
