Home First are excited to be able to advertise a Band 6 Physiotherapist post. Working to support the Physiotherapy needs of patients and staff on our Inpatient pathway.

Home 1st is a multi-disciplinary integrated service encompassing health and social care the focus of which is to work collaboratively with other local providers for the purpose of admission avoidance and supported discharge pathways for the residents of Milton Keynes.

Providing Physiotherapy assessment to enable access to inpatient and community rehabilitation services to adults in Milton Keynes, and to community admission avoidance beds as well as supporting the prevention of admission to acute services.

Providing a varied learning experience and the ability to develop your career within a community setting. Working in co-operation with the wider multi-disciplinary team and offering comprehensive assessment, advice, treatment and discharge planning in order to maintain patient flow. Opportunities to work across the acute and community settings and experience a wide variety of clinical conditions are always available.

Main duties of the job

  • To provide Physiotherapy assessments and deliver person centred rehabilitation and individualised goals.
  • To undertake all aspects of clinical duties as an autonomous practitioner.
  • Working in rotations within the Community,assisting in admission avoidance assessments, the Getting People Home pathway to facilitate early supported discharge from the acute hospital and Inpatient rehabilitation pathways.
  • To communicate effectively and work collaboratively with all members of the multidisciplinary team and other services within Home 1st, as well as outside agencies.
  • Ability to work in capacity as a Trusted Assessor.
  • Participate in development of integrated pathways.

Job description Job responsibilities

CLINICAL
  • To be professional and legally accountable for all aspects of your work including the management of patients in your care.
  • To use specialist professional and clinical knowledge across a range of procedures based on a sound knowledge of evidence based practice and treatment options, using clinical assessment, reasoning skills and knowledge of treatment skills.
  • To undertake assessment of patients, using clinical reasoning skills and manual assessment techniques in order to diagnose clinical need.
  • To assess patient understanding of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.
  • Ensure the participation of patients families and carers in decision making and planning mutually agreed objectives.
  • Use a range of verbal or non-verbal communication tools to communicate effectively with patients, relatives carers and other health and social care professionals to progress rehabilitation and treatment This will include patients who may have difficulties in understanding or communicating.
  • To manage clinical risk within the Home 1st therapy caseload.
  • To be responsible for the safe and competent use of all appropriate equipment.
  • Work collaboratively with the MDT with discharge planning.
  • Ensure accurate electronic records are maintained timely and effectively.

PROFESSIONAL
  • To comply and uphold standards of the HCPC and the Chartered Society of Physiotherapy.
  • To be responsible for maintaining own competency to practice through CPD activities and to maintain a portfolio which reflects personal development.
  • To be responsible for attending and participating in formal supervision, including clinical supervision, in accordance with Trust policies and procedures and the standards of the CSP.
  • To participate in the staff appraisal scheme as an appraisee and be responsible for complying with your agreed personal development programmes to meet knowledge and competency requirements.
  • To undertake the measurement and evaluation of your work and current practices through the use of evidence based practice projects, audit and outcome measures in conjunction with the Home 1st Lead Physiotherapist.
  • To be an active member of the in-service training programme by attendance at, and participation in, in-service trainingprogrammes, tutorials and individual training sessions.
  • To supervise designated team members and complete appraisals, maintaining accurate documentation.
  • To be responsible for planning own caseload, prioritising workload as situations arise.

PERSONAL RESPONSIBILITY

  • Carry out assessments and manual treatments of patients with moderate physical effort on a daily, prolonged basis.

  • Be responsible for the health, safety and welfare of patients, staff and visitors, ensuring that work practices conform to the Health & Safety at Work Act. To report without delay, accidents and incidents, both verbally and in writing as per department and Trust procedures.
  • Attend mandatory training in accordance with Trust policy and any other relevant training that is required in order to satisfactorily undertake the duties and responsibilities of the job.
  • Respect the individuality, values, cultural and religious diversity of patient and contribute to the provision of a service sensitive to these Ensure that all actions promote equality and diversity in accordance with CNWL policy and the law.

Person Specification Qualificatins & Experience Essential
  • BSC Degree in Physiotherapy
  • HCPC and Chartered Society of Physiotherapy Membership
  • Evidence of Continuing CPD
  • Post qualifying experience in relevant clinical setting
  • Experience of Community/Rehabilitation/Acute/Stroke
  • Effective written communication skills
  • Confidence in basic IT skills
