Due to the expansion of the homeless hub within Greater Manchester Mental Health Trust there has been funding for a Dual Diagnosis Practitioner for the Salford Dual Diagnosis Homeless Team. The post holder will work within an integrated, multi-disciplinary homeless Dual Diagnosis team within Greater Manchester Mental Health Trust in addition to statutory and voluntary agencies providing support homeless people in Salford, focusing on those who have suffered domestic violence. The successful candidate will work in partnership with other providers, to engage identified individuals who may present with co-occurring addiction needs and mental illness alongside physical health issues, and multiple, complex social needs.

The Dual Diagnosis Practitioner will work along side hospital teams, community teams and tenancy support officers working with this cohort. The role will require a high focus on multi-agency working, liaison, signposting, onward referring, and providing advice and consultation around homeless health needs.

Main duties of the job

The Dual diagnosis Practitioner will focus on providing assessment and support to this service user group. This may include low-level interventions, engaging individuals with local service provision, ensuring they are able to access the support they need to maintain engagement with care and treatment as identified, enabling them to overcome barriers that prevent them from engaging with the health and social care system and also supporting other providers to understand the impact of dual diagnosis for the individuals and also upon service engagement and support approaches.
