The Sheffield Memory Service offers the opportunity for experienced, enthusiastic, open minded nurses and/or occupational therapist to provide assessments and post diagnostic interventions and advice to people over the age of 65 who are experiencing memory difficulties. You will need to be prepared to dip your toe into new ways of working leaving the confines of your professional discipline and embracing the unknown. Together we can enable and empower services users to live well with dementia.

Main duties of the job

You will need to have 3 years post graduate experience in mental health, with evidence of dementia experience and knowledge.

You will need to have excellent IT and typing alongside your time management skills.

You will need to be able to work under time constraints in our busy out patient department and enjoy the process of talking with service users and their supporters (carers) to gather an assessment report to inform the diagnosis process.

You will enjoy learning new skills and the challenges of delivering educational interventions either on a 1:1 or group basis.

But essentially, you will enjoy working closely within the multi disciplinary team, sharing new learning and service delivery both in hub and in a community setting.

Job description Job responsibilities

  • Actively contribute to the development and delivery of therapeutic group work and 1:1 interventions
  • Develop and ensure each allocated service user has a comprehensive, collaborative up to date treatment plan, with specified objectives designed to meet the individuals identified needs, and ensure that effective communication occurs with other relevant parties
  • Ensure consent is gained for all participants in the clinic by communicating to both the service user and carer the purpose of the appointment and provide relevant details to ensure an informed decision regarding participation can be made
  • Communicate the diagnosis/outcome of the assessment in post diagnostic appointment to develop a collaborative informed plan to enable and empower the service user and their significant other/carer to live well with dementia
  • Review and discussion medication as appropriate
  • Identify and advise the most effective method of referral to other agencies/services to support the plan
  • Take an active role in the Case Management Team providing appropriate support/advice and decision making for service users and their significant other/carer on the dementia register
  • Actively participate in the continued improvement and development of the service

  • RMHN qualification with 3 year post grad mental health working experience
