Community Nurse & Health Facilitation Nurse Learning Disabilities

An exciting opportunity has arisen for a skilled RNLD to experience working as part of our Community Learning Disabilities team in Coventry & Rugby.

The role is unique in that it combines Community LD Nursing role with Specialist Health Facilitation role for the team. You will be working closely alongside other Community Nurses as well as the Acute Liaison team.

The Community Nursing role involves close collaborative working with multi-disciplinary colleagues in the community setting, managing own caseload, supervising others, completing assessments, raising awareness of the Mental health needs of adults who have a learning disability, identifying unmet physical health needs and providing specialist nursing advice.

The Health Facilitation aspects of the role will include improving links with primary care services including hospitals/ G.P surgeries, provide support, information and education to clients, carers and provider's and also deliver education to people with learning disabilities around health needs both on 1:1 or group basis

The successful candidate will need to be proactive, self-motivated and have experience of autonomous working, whilst also working under the direction of the community team manager. They will be able to use current best practice to gain the best outcomes for Clients and their Carers.

Essential Requirements include:

RNLD and current NMC RegistrationExcellent communication skills

Experience of working with people with a learning disability in the community in a community nursing and/or health facilitation role

Driver with access to independent means of transport


Person Specification Qualifications Essential
  • RNLD Degree or Diploma
  • Teaching/ Mentoring Qualification
  • Evidence of ongoing CPD
Other Essential
  • Able to travel for work purposes and have access to an independent means of transport.
Knowledge and skills Essential
  • Knowledge and understanding of up-to-date legislation in respect of Learning Disability Service
  • Demonstrate understanding of responsibilities of registered nurse according to NMC advisory codes
  • Able to understand and contribute to services strategic framework and service visions
  • Highly effective communication and liaison skills
  • Ability to manage own workload and to work in an autonomous capacity
  • Clinically proficient and able to provide direct assistance to clients with complex needs
  • Computer literacy in Word, PowerPoint and excel.
Experience Essential
  • RNLD with a substantial post registration experience at Band 5.
  • Experience of working in multi agency teams with multi-disciplinary colleagues.
  • Experience of working as a community nurse (With people who have a learning disability)
