We have an exciting opportunity for a full-time (1.0 wte / 37.5 hours per week) Community Learning Disabilities Nurse in our well established Wakefield Adult Community Health Learning Disability Team providing a service to the whole of the Wakefield area, your base will be central to the area however, you will work in an agile manner.
The Team consists of a comprehensive supportive multi-disciplinary team comprising of psychiatrists, clinical psychologists, learning disability nurses, occupational therapists, physiotherapist, speech & language therapists, dietician and healthcare support staff. The Team works together to support adults with learning disabilities and their families/carers across Wakefield.
The Team are passionate about their work and reducing health inequalities for people with a learning disability.
You will need to demonstrate an ability to work autonomously and as part of a wider team. You will manage a case load and supervise junior staff with a delegated caseload and support them in their development.
The successful candidate will have a good understanding of the complex needs of people with learning disabilities and be able to demonstrate that they have the qualities to deliver safe, effective and personalized care to meet the needs of the service user. You will be committed to improving outcomes for our service users and have excellent interpersonal and communication skills. You will be providing expertise and skills to identify the health needs and support required in the provision of care as well as providing health promotion to support and empower people with a Learning disability.
You must hold the relevant NMC qualification.
