Community Mental Health Nurse, Band 6 Permanent Full Time: 37.50 hours per week

We have an exciting opportunity for an experienced nurse to join our ADAPT in Bexleycare Community Mental Health Team. The team provides an experience to work in a busy and varied environment, alongside clinical and other non-clinical colleagues to make a positive contribution to our patients and staff experience.

If you are interested we are looking for a motivated, skilled, and experienced Nurse to join our Community Mental Health Services in Bexley ADAPT Team.

Main duties of the job

The post holder will be committed to improving the mental and physical well being of mental health service users that are accessing care in our team.

The post holder will be responsible for a caseload of service users who have complex and substantial mental health needs. They will work as a member of the MDT ensuring that high quality individualised care is delivered and that this promotes recovery and maximise independence.The post holder will be responsible for undertaking comprehensive assessment of service users and carers mental health and social care needs, in accordance with National CPA Guidance.

The post holder will be working in a supportive diverse team, where there are opportunities for personal development.

  • Mentorship qualification
Desirable
  • Evidence of further training in the last year
Experience Essential
  • Experience working in a clinical setting with people with complex mental health needs.
Desirable
  • Previous experience working in a community setting
Skills Essential
  • Experience in undertaking a variety assessments with people with mental health needs,
  • Excellent communication skills
  • Manage and create care plans for a caseload of patients with complex needs
Desirable
  • Skilled reflective practitioner.
  • Experiencing of working with trauma, ASD , EUPD diagnosis
