As part of the 10 year NHS plan to reform the way we deliver mental health services the Darlington Locality we are looking to recruit 2 x Senior Registered Practitioners (RMN/Social Worker/OT) that will be primarily based in the front end of the service. This role will be a vital part of the new ways of working with a collective team working together with our other colleagues in the community to support patients receive the right service from day one. Integrating services around the needs of the person minimising duplication and enabling services to work together.

The Senior Practitioners in the HUB will be able to provide excellent assessment skills, interpersonal and team working, and be able to work collaboratively with a range of statutory and third sector agencies.

The successful applicants will work with a range of agencies locally to facilitate safe care and treatment. Experience of multi-agency working would be helpful. A keen ability to network well and interact with professionals, carers and patients across multi-agency settings is essential.

This post will initially be based at West Park Hospital, however we are looking to acquire alternative premises in the community setting. The aim is to have a Hub location that is easy accessible and convenient for the people will who work with and alongside.

The Access Practitioner job description gives an overview of the current expectations, experience and skills required. but this role will be developed within the new model and will evolve to be part of the bigger Hub Team, and as such their will be opportunity to complete a variety of work. The Hub will provide the gateway to a variety of services that will be developed by the Senior Practitioners working in the team.

We welcome Registered RMN/Social Worker/Occupational Therapist - you must be able to demonstrate a knowledge, skills and experience of assessment and treatment of Mental Health conditions over a period of time. As this post will involve having a high level of assessment skills, it is essential to have experience in this aspect of care.
