Oxford Health's Buckinghamshire Mental Health Directorate is working in collaboration with Buckinghamshire's Primary Care Networks (PCN) to develop and implement improved access to specialist mental health care and treatment within primary care services. Focusing onbridging the gapbetween primary and secondary care Mental Health services for individuals withcomplex and severe mental illness.

The service is designed toempower patients 18years+to access specialistmental health support locally, within their GP Surgeries in a timely manner.

Main duties of the job

The successful Social Worker will facilitate improved opportunities for joint workingwith our voluntary care sector, contributing to theleadershipand development of national mental health agendas within their Primary Care Network (PCN). As part of the Primary Care Networks Multi-Disciplinary Team (MDT) your role will provide mental healthtriage, assessment and adviceto patients, carers, and colleagues. Facilitatingearly interventionstoaid recoverywithin the community and utilising your profession specific skills toenable prompt access to specialist servicesfor patients.

As the Social Worker in this post, you will be expected to use your profession specific skills and values in this role and apply Social Work theories to practice, such as rights and strengths based approach, alongside your expert knowledge of the Care Act eligibility and safeguarding principles to support your clinical practice and signposting to appropriate services.

Job description Job responsibilities

  • To provide a lead role in supporting the PCN in triaging mental health concerns raised by on the day patient requests, highly providing specialist advice and support within GP surgeries
  • To review patients presenting with mental health issues, consider a range of options regarding treatment interventions providing them with highly specialised advice concerning care when appropriate and liaising with GPs and MH teams
  • To offer brief interventions as needed eg problem solving, decision making, building resilience, building on personal strengths, risk de-escalation.
  • Identifying care act eligibility and referring appropriately for assessments, carers assessments or safeguarding concerns.
  • Working as a member of the primary Care multi-professional team and taking responsibility for the development and implementation of specialist mental health training to the PCN.
  • To provide a navigation role to help GPs make appropriate referrals to the relevant community and secondary care services bridging the gap between primary care services such as IAPT and secondary care, working in partnership across the system.


  • Person Specification Qualifications Essential
    • Current professional registration: Registered Mental Health Nurse, Social Worker, Psychologist, Occupational Therapist
    • Evidence of relevant post registration training and development in particular motivational interviewing, brief solution focused interventions, cognitive and behavioural approaches, and psychosocial interventions
    Desirable
    • Recognised Post Graduate clinical or leadership qualification relevant to the role
    • Practice Educator/ AMHP/ Best Interest Assessor
    • Assessed Supported Year in Employment/ or Equivalent
    Knowledge Essential
    • Advanced understanding of the mental health needs of adults
    • Specialist knowledge of person-centred care and its application
    • Able to demonstrate advanced understanding of health inequalities amongst marginalised groups
    • Specialist knowledge of psycho-social interventions and their appropriate use
    • Working knowledge of appropriate assessment tools
    • Working knowledge of the policies and guidelines pertinent to the planning and delivery of community mental health care to adults with mental health needs
    • Recovery focused practitioner able to apply recovery principles to adults and older people, understanding of 'strengths- based' approaches
    Desirable
    • Knowledge of practical application and use psychological therapies
    • Awareness of the impact of the health economies on service planning and delivery
    • Specialist knowledge relating to MH needs of young adults age 18-25 and to the needs of those diagnosed with, or with presenting traits of, personality disorder
    Experience Essential
    • Community experience in mental health services for adults and older age adults with severe / significant mental illness
    • Experience of providing a range of evidence based interventions to people with a variety of mental health problems and serious mental illness
    • Considerable experience in the assessment and management of clinical risk.
    • Experience in physical health assessment for people with mental health needs.
    • Experience of effective collaborative working in partnership to deliver holistic health and social care outcomes
    • Evidence of Interdisciplinary / multi-agency working experience including primary care and community/ voluntary sector.
    • Experience of leadership and development of junior colleagues.
    • Significant and extensive practice experience post registration working with people with mental health needs
    Desirable
    • Proficient in clinical audits and research.
    • Experience of working in primary and secondary mental health services.
    • Experience in the planning and delivery of training and practice developments to staff.
    • Experience in service evaluation and improvements.
    • Experience working in a CAMHS and/or OPMH service
