An exciting opportunity has arisen within the Mental Health Urgent Care Division to work as a senior mental health practitioner to deliver high quality safe and effective care.
This is a demanding and challenging role where, as a successful candidate, you will be required to demonstrate high levels of resilience, excellent problem solving and inter-personal skills as well as the ability to provide sound clinical and managerial support to team members.
The post holder will also demonstrate a passion for mental health urgent care team delivery, quality improvement and attitude which respects and values service users' choice and works alongside them and their carers to ensure they are at the centre of care planning.
This Post is open to Annex 21 applicants wishing to develop within the area of Mental Health Urgent Care covering Home Treatment, Assessment/ Urgent Response/Crisis Line and Core 24 (AED's) across the division.

Main duties of the job
The post holder is responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care within the framework of Effective Care Co-ordination (ECC).
The post holder will operate as part of an Mental Health Urgent Care Service but be expected to be flexible in terms of delivering care within a variety of environments.
The team will work to assess and provide treatment to service users with complex needs. Alternative options to admission will be considered where possible.
The post holder will demonstrate an attitude which respects and values service users' choice and works alongside them and their carers to ensure they are at the centre of care planning.
The post holder will be expected to participate in a 24hr roster system 7 Days a week, on a rotational basis as and when the service demands.

Job description Job responsibilities CLINICAL:
Formulate, contribute to, and participate in, the development and implementation of assessment and care planning systems in collaboration with the multidisciplinary team
Manage a case load within the area appointed
Carry out generic and specialist assessments, including risk and provide therapeutic interventions/ home treatments within the most appropriate setting in accordance with the service users and/or carers needs. Complete Gatekeeping for all Inpatient admission requests across the division. Assess whether users can be managed safely within the community promoting quicker recovery. Assess Risks and collaboratively develop safety plans with users/carers. Ensure effective liaison with inpatient units/AED's across the Trust to facilitate early discharges or to undertake reviews of those users placed out of area. Act as the Duty Practitioner /2nd Duty, as part of a rotational roster.
Provide signposting and support with onward referrals to third sector services
Fully participate in ECC undertaking a care-coordinator role as appropriate
Assist in the development and implementation of integrated care pathways, including appropriate service user assessments/referrals and models of care.
Ensure record keeping is in accordance with Professional and Trust standards.
Ensure the safe management of medicines and adhere to safe practice and principles of administration in accordance with the NMC code of professional conduct and Trust Medicines Code.
Work in a professional and collaborative manner with the multi-disciplinary team.
Participate and promote positive intra-agency working.
Responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care without supervision and act as Named Nurse as required.
Participate and assist in setting standards of service user care based on sound research findings.
Act as knowledge resource in specialist area of practice to other team members and other agencies as required.
Where indicated and following completion of the relevant training the practitioner will act as a non-medical prescriber, at all times working within their sphere of clinical competence. MANAGERIAL:
Deputise for the Clinical Team Manager as required (dependant on Annex 21 application)
Undertake delegated responsibilities as agreed with Clinical Team Manager.
Support the formulation and implementation of clinical and operational practices, and procedures as appropriate.
Support the implementation, monitoring and updating of an operational policy and philosophy for the team.
Actively participate in service development
Ensure the principles of Clinical Governance are incorporated in service delivery by maintaining a high quality service based on agreed clinical standards.
Ensure adherence and implementation of Effective Care Coordination.
Assist in the maintenance of an accurate up to date information system and routinely provide information relating to clinical activity.
Ensure that Trust policies and procedures are adhered to.
Assist in the development of local protocols to ensure a high standard of service delivery is achieved and maintained.
Support the process of staff participation in the formulation of new ideas/suggestions to improve service user care and service delivery.
Assist in the maintenance and updating of the teams equipment and service schedules, and ensure regular maintenance, repair or replacement as required adhering to Health & Safety, COSHH and Medical Devices Agency regulations.
Ensure effective processes of communication are established, maintained and regularly evaluated within designated team.
Assist in the investigation of complaints and incidents in accordance with Trust policy.
Participate in Serious Incident investigations/reviews as required
Participate in managerial supervision.
Appropriate participation in Managing Performance of staff as required. EDUCATION AND DEVELOPMENT:

Support the process to ensure all relevant staff have an up to date Individual Personal Appraisal and Personal Development Plan.
