Durham City East Community Mental Health Team would like to hear from qualified practitioners with an interest in mental health.

You will be working as a care coordinator within a newly developed community team, and will be part of a supportive, welcoming and friendly environment.

The post is open to qualified nurses, social workers and occupational therapists.

Newly qualified or due to qualify practitioners are also welcome to apply!

Main duties of the job

You will provide support to individual service users, their carers and wider family. This role will involve direct work with adults with a variety of mental health needs.

You will use the generic combined pathway to support your assessment and treatment.

You will receive regular supervision and caseload management and have support from various professional disciplines within the team.

Person Specification Qualification Essential
  • RNMH/LD or Social Worker or Occupational Therapist
  • Evidence of Continuing Professional Development.
  • Key skills in literacy, numeracy and ITQ level 2 (or equivalent)
  • Post-graduate qualification in a relevant area
Experience Essential
  • Working with adults with mental ill health
  • Working in a person centred therapeutic manner
  • Working in a community setting
Knowledge Essential
  • Evidenced based knowledge of best practice in caring for patients in the designated field.
  • Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act).
  • Care Programme Approach and its application in practice.
