We're offering an exciting opportunity to work with the Dover and Deal Community Mental Health service for older people (CMHSOP). We're looking for a band 5 community mental health nurse with an interest in dementia care, to work with clients referred to our memory service. The memory service provides assessment, diagnosis and aftercare including medication reviews, adapting to a new diagnosis and signposting to appropriate community services whilst working closely with carers and loved ones.

The team also support people who have reached older age and are experiencing mental health difficulties such as depression, psychosis and complex emotional needs, in addition to specialist interventions for people with a diagnosis of dementia that have complex needs.

Your role within the team would be varied allowing plenty of scope to both demonstrate and develop your skills as well as offering a dynamic working life. Your main focus would be within the memory pathway but will include the opportunity to work closely with our senior staff in the functional and complex needs pathway monitoring mental health, risk and co-ordinating treatment plans.

As a band 5 clinician you will be working autonomously but you will have access to the support of admin, mental health support workers, psychologists, consultant psychiatrists, nursing and occupational therapy staff. We work very closely as a team providing plenty of opportunity to discuss clinical cases and risk.

Main duties of the job

  • Conducting initial assessments of people with memory and mental health needs
  • Providing after care to people recently diagnosed with dementia
  • Being part of the duty team, as required
  • Managing a caseload of clients
  • Being part of team meetings and MDT discussions
  • Supporting and being a role model for junior colleagues
