Transfer of Care: Community Physiotherapist. Band 6

The Transfer of Care Team facilitates the streamlined discharge of an individual from hospital to home utilising the Discharge to Assess Model. The team also supports the prevention of admission from the community alongside the Urgent Care and Response Team.
South Bedfordshire Community Health Services are looking for a 30 hour/4 days a week band 6 Physiotherapist to be part of the Therapy Team. We are a 7 day service 8am-9pm.
Main duties of the job
You will need to have strong assessment skills with a personalised approach, have the ability to carry out Physiotherapy assessments of people with a range of needs. After the assessment, you will then develop, implement, manage and review the planned recommendations. You will need to have skills in moving and handling, risk assessing and identifying solutions to reduce risk to provide for the needs and personal outcomes of the individual, their families and carers.
Physiotherapists play a key role in shaping and developing the service provided and with this there will be opportunities to develop the role and service in a collaborative approach with Social Care colleagues and the acute settings.

To undertake comprehensive assessments of clients who may have complex conditions including multi pathology. Use well developed clinical reasoning to select appropriate assessment techniques.
To analyse the information gained from the assessment to formulate an accurate physiotherapy treatment plan for the client and then discuss and agree goals and outcomes for treatment with them. To set realistic timescales for achieving these.
To select and be able to perform appropriate treatment techniques from a wide range of specialist skills. The selection of techniques will be based on both theoretical and practical knowledge and evaluated past experience.
To demonstrate the physical ability to carry out physiotherapy assessment and interventions.
To be responsible for all aspects of safety relating to equipment used. To be responsible for checking and/or teaching competency of more junior therapists and non-qualified staff in using equipment e.g. appliances, gym equipment, electrotherapy items etc.

- Masters Degree
- Member of the Chartered Society of Physiotherapy
- Broad range of physical experiences clinically
- Willing to give presentations/lead teaching sessions for colleagues
- IT Skills (word, e-mail, excel, virtual platforms)
- Community setting experience
- Supervision experience
- Experience of working within the NHS as a physiotherapist
- Detailed understanding of a wide range of medical conditions.
