We are looking for an Advanced Practitioner in Social Work to provide professional leadership to social work colleagues working in the East Hull Locality Mental Health Team. The team is multi-disciplinary and delivers a range of high-quality health and social care interventions across primary and secondary care settings.

Main duties of the job

The post-holder will support the team to work in co-production with a range of statutory and non-statutory partners, people who use our service, and their carers in order to maximise access to community and wellbeing resources in support of person-centred recovery planning.

This will include supporting social workers with, and undertaking person centred assessment, planning, implementation, and evaluation of appropriate actions, to ensure that resources are utilised effectively to empower the people who use our service and to safeguard and promote their welfare.

The post-holder will:

provide high quality supervision to Social Workers andact as a practice educator for students and newly qualified social workers undertaking the Assessed & Supported Year in Employment

apply professional leadership, expertise, skills and advice to raise and maintain standards of professional social work practice,

ensure a safe effective and high-quality service that meets agreed policies and practice requirements and delivers key social care agendas within a multi-agency context.

take a lead role in developing strong links with primary and secondary care statutory partnership organisations, and the voluntary sector in order to offer an integrated response to people who use our service and carers, which promotes independence and empowers individuals to develop their own recovery plans and to prevent the need for ongoing involvement with statutory mental health services in line with the Care Act.

  • Educated to Masters Level or equivalent

Experience Essential
  • Evidence of advanced practice skills and able to demonstrate the impact of this on practice change/development
  • Demonstrable experience of working in the specific field where the post is held
  • Evidence of sound post-registration professional practice
  • Demonstrable experience of working in the specific field where the post is held
  • Ability to work across team/organisational boundaries developing and maintaining multi-professional and multi-agency partnerships
  • Experience of developing others through education, mentorship, coaching, teaching, assessing, presentations, publishing
  • Proven leadership/management experience, able to demonstrate examples of positive cultural change in service delivery/practice
Knowledge Essential
  • Full understanding of relevant policy, legislation, drivers and their application to professional and service area, e.g. Safeguarding, Care Act, Mental Capacity Act and Mental Health Act
  • Advanced/expert understanding of relevant practice standards and protocols within identified practice area.
  • Full understanding/application of relevant professional practice/standards
  • Member of specialist interest group/network, regionally or nationally
