We are looking to recruit three nurses. This is an ideal opportunity for nurses who are are interested in developing to Band 6 after 18 months.
We aim to recruit a motivated, committed crisis response and home treatment practitioners for the East Surrey Home Treatment Service
The post is for full time however part time option are also available.
This opportunity is open to registered mental health nurses (NMC)
The post holder will be an autonomous practitioner who can also work effectively as team player. You will need to demonstrate excellent communication skills and be able to work collaboratively within a multidisciplinary team. You will be committed to the delivery of a high-quality service, working both independently and as part of a well-established, innovative and crisis response and home treatment team in meeting the needs of service users and their carers.
Qualified nurses are currently benefiting from Retention Allowance. It is a 10% pay uplift on the top of HCAS Fringe payment. This Premium is subject to a minimum total payment of £3,553 and a maximum total payment of £4,528 p.a. This will be paid on a monthly basis and pro-rata to hours worked
The candidate must have a full and valid driving licence and access to a vehicle for work purposes due to the community element of this role.
Main duties of the job
To be a core member of the multi-disciplinary team, participating in the provision of on-going assessments, care planning, treatment and support of people with serious and enduring mental health problems and people in crisis to promote health, independence and recovery.

Key Responsibilities Band 5
• To discharge professional responsibilities under the Mental Health Act (1983) and revised subsequent act Mental Capacity Act (2005) and the NMC Professional Code of Conduct
• To provide nursing care across the home treatment team, assessment suite (hospital based place of safety) and safe havens, as required.
• Be accountable and responsible for a defined caseload that reflects the priorities of the local population and carry responsibility for the assessment of care needs; under the supervision of the Clinical Lead.
• To develop, implement and evaluate evidence based programmes of care; set standards of care and liaise with other agencies, particularly in relation to people discharged under the Care Programme Approach.
• Participate in the multi-disciplinary assessment of individuals with mental health problems living in the community, by providing a community mental health nursing perspective and expertise
• Ensure that a risk assessment is completed for service users, especially if there is a history of, or a potential for risk. Escalating to relevant person or multi-disciplinary teams where there are concerns, or assessment is out of personal scope of competency
• To be aware of the potential for vulnerability to abuse amongst the service users/families, and use of the local policy to inform appropriate personnel in line with safeguarding vulnerable adults and children and SABP Policies
• Ensure that carer’s are identified and referred onto the local authority for a carer’s assessment.
• Help develop and maintain effective joint working arrangements with colleagues in the community mental health services, residential services, primary health care, social services and voluntary services, thus facilitating the co-ordination and comprehensiveness of provision.
• Utilise the full range of mental health nursing skills to participate in the assessment, care and support of service users.
• Use specialised knowledge, experience and skills in direct clinical work and act as a resource for the team and other agencies. This may include work in rehabilitation, social interventions, development of social networks and accessing other services/resources, mental health promotion, counselling and intercultural therapy, encouraging service user participation at all times.
• Participate in the provision of community mental health nursing advice and consultation to colleagues, community groups and organisations.
• Promote the recovery model to the people who use the service and their carers
• To act in a manner to respect the customs, individuality, values, sexuality and spiritual beliefs, actively supporting the individual to fulfil these.
• Ensure detailed and appropriate records are maintained to demonstrate that contact with people who use services, and their carers’, is meaningful and effective.

Person Specification Qualifications Essential
- Experience required
- NMC Registration as RN3 or RNMH
- Band 5 1 years experience working with people with mental health (can be prequalification) or at least one placement during final year of training in a Home Treatment or Community setting
- Band 6 18 months post qualification experience of working in a home treatment and/or
- community team working with people with severe and enduring mental health illness
- NMC Registration as RN3 or RNMH
- Suitable for someone who is passionate about providing a high quality holistic approach to the care and support provided to clients in the community who have been identified as in need of community nurse support to enable them to exercise choice over their own lives.
