Are you a Registered Mental Health Nurse, Social Worker, Occupational Therapist?

We are seeking registered, motivated mental health professionals to work in our expanding Home Treatment Team (previously known as the Crisis Service) based in one of our 4 teams across Somerset (Yeovil, Taunton, Wells, Bridgwater).

You will act as a key member of the multi disciplinary team, liaising with statutory services and other agencies. You will be part of a dynamic team, who are responsible for ensuring rapid community assessments of mental health and risk. You will have sound assessment skills and provide brief interventions to clients as an alternative to admission.

Main duties of the job

We recognise that recovery from acute mental illness is best met within a persons own home, so we strive to provide safe, intensive mental health treatment and intervention at home as an alternative to admission to psychiatric hospital.

You will be a key member of a skilled Multi-Disciplinary Team who will offer a wide range of psychological, medical and social interventions to support recovery

As well as working with a clinical caseload and their families, the home treatment teamgatekeep admissions to and facilitate early discharge from psychiatric inpatient settings

This role will involve flexible working across a 24-hour shift rota (days and night shifts) in a 7 day a week service.

  • The team act as a gatekeeper to admissions so the post holder will be able to consider referral information from other services and be able to contribute to decision making regarding the most appropriate treatment option.
  • The post holder will also work in our triage hub, responsible for accepting and triaging urgent referrals from a variety of sources, for those experiencing a mental health crisis.
  • You will be expected to maintain a high standard of electronic patient records.
  • You will work collaboratively with colleagues in the team and across the community mental health service. There will be an expectation that they will work flexibly across the service which covers a 24-hour period, 7 days a week.
  • We are looking for someone who is professional in how they communicate, work with care and compassion, are competent, committed, courageous, reliable, trustworthy and dependable in undertaking allocated tasks and responsibilities.
  • The service has seen many positive developments in a short period, which we continue to play an important role in- supporting individuals in a Step- up House as an alternative to hospital, and having close working links with the Mindline phone and Crisis Safe Space services in the county. These collaborative and integrated working models are part of an exciting future for the people of Somerset in what is available to support their Mental Health.


Person Specification Qualifications/Training Essential
  • RMN, Social Work or Occupational Therapy Qualification
  • Current registration with relevant professional body
  • Evidence of post qualifying professional development in an acute setting
Knowldege and Experience Essential
  • Significant post registration experience in multi-disciplinary community mental health services
  • Experience of managing risk of clients in crisis
  • Experience of assessment, care-plan, development and evaluation in conjunction with clients and carers
Desirable
  • Experience of mental health difficulties
  • Good knowledge of resources and working practices across health and social care
  • Ability to be innovative in practice
Personal Qualities Essential
  • To be able to demonstrate an awareness and responsibility whilst recognising the impact frequent exposure to distressing circumstances has on care and compassion.

  • Excellent communication and interpersonal skills including presentation of cases to multi-disciplinary case conferences.
  • Able to carry out evidence based interventions.
  • Compassionate - Exceptional interpersonal skills with the ability to communicate effectively with patients, carers and relatives about complex and sensitive issues remaining sensitive and empathetic.
  • Excellent organisational skills, ability to manage own time and plan timed activities for staff and patients
  • Ability to recognise and manage challenging situations in a calm and professional manner.
  • Ability to lead a team, demonstrating teaching and mentorship skills and work effectively as part of a team.
  • Experience of carrying out supervision to junior staff, coordinating the care of clients and delegating tasks.
  • High standards of written communication skills with the ability to use email and internet.
  • Ability to undertake PMVA training to required level for role.
Desirable
  • Ability to work in partnership with other agencies.
  • Willingness to lead groups
  • Experience of multidisciplinary working in a community setting
