The Role:
You will be assigned to the Four Counties PCN to support the community mental health transformation programme and will work autonomously providing mental health leadership and implementing clinical processes.

Whilst a full Job and Person specification is attached to this advert, key elements of the role will include (but not be limited to):

• Offering clinical leadership to a team of professionals from a variety of background and skills to support people in the community with the mental health and wellbeing.
Developing and implementing clinical processes that contributes towards the continuous improvement of mental health provision across the Four Counties PCN (multiple GP practices)
• Providing best practice training, mentorship and guidance to mental health practitioners and other roles within the primary care/ neighbourhood MDT
• Managing presentations of highly complex/SMI presentations working closely with the secondary mental health services, to maintain personalised care plans which focus on what matters to the person and avoid escalation to urgent care pathways

The Successful Candidate will:
• Have the qualifications and experience as highlighted in the Person Specification.
• Be an experienced clinician with proven skills in working with people who have a range of highly complex mental health needs, demonstrating practice that is evidence-based and innovative.
• Have some experience of working closely with or within primary care.
• Demonstrate adaptability, excellent communication skills and self-motivation.

The post holder will be encouraged to contribute to service development, and there will be opportunities for CPD and clinical supervision both via psychiatry department and PCN.
