The Learning Disability Intensive Support Service (LDISS) is a nationally recognised service, it provides people with a Learning Disability with behaviours that challenge the service or a mental health condition that cannot be supported in mainstream services, the required support in the community with the aim at keeping people at home and out of hospital.

LDISS aims to give service users a meaningful, high quality and unrestrictive life where possible. Through providing the right interventions and support, service users will be assisted to live a life where they have choices, are included in their communities and achieve their potential.

As LDISS works with some of the most complex individuals of all ages with a learning disability in the county, this will frequently require a highly skilled and imaginative approach to achieve a good outcome. LDISS will focus on social activities, inclusion, life skills, friendships and family relationships as well as providing professional assessments, skills and guidance.

Main duties of the job

We require someone who has experience of working with individuals with Learning Disability and or Autism, who are committed to continuing to develop the high quality services that our service users deserve. If you are proactive in working with carers and families and have a robust and sensitive demeanour we would like to hear from you.

The post will work alongside other members of the Learning Disability and Mental Health service, including the CLDT Managers, Crisis and Commissioning and County Council Colleagues.

You will be part of the Clinical Team and provide clinically effective and evidence based services in the prevention and management of aggressive incidents. The hours of work are 37.5 per week and duties will involve flexible working potentially across a 24 hour rotational shift pattern.

Job description Job responsibilities

We are looking for an experienced practitioner to bring their expertise to our team. The post holder will work with individuals, staff team, families and carers, as a leader within the MDT and will implement, monitor and model interventions that will increase the individuals quality of life. The post holder should have a knowledge of PBS and Active Support and be able to analyse data and feedback the results to the wider team. The post holder will need to demonstrate enhanced competence in decision making, clinical reasoning and the management of clinical needs and have experience in working with individuals who require an individualised and inspirational approach.

This post requires the post holder to be able to undertake all statutory and mandatory training and post holders must be able to undertake physical interventions. Fitness for physical intervention will be assessed by Occupational Health.
